摘要
目的:探讨吸入用布地奈德(BUD)联合口服孟鲁司特钠(MON)、左西替利嗪(LCT)治疗小儿咳嗽变异性哮喘(CVA)的临床效果。方法:选取2019年1月—2021年12月锦州医科大学附属第一医院收治的100例咳嗽变异性哮喘患儿作为研究对象,随机分为观察组与对照组,各50例。对照组给予BUD气雾剂吸入治疗,观察组在对照组基础上应用MON及LCT治疗。对比两组临床疗效、血清炎性因子水平、肺功能指标、不良反应发生情况。结果:观察组治疗总有效率高于对照组,差异有统计学意义(P<0.001)。治疗后,两组血清免疫球蛋白E、白细胞介素-4、白细胞介素-6水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.001)。治疗后,两组第1秒用力呼气容积、肺活量、第1秒用力呼气容积与肺活量比值、动脉血氧饱和度均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.001)。观察组不良反应发生率低于对照组,差异有统计学意义(P=0.029)。结论:吸入用BUD联合口服MON、LCT治疗小儿CVA的效果显著,能降低炎性因子水平,改善肺功能和动脉血氧饱和度,且不良反应发生率低,安全性高,值得临床应用。
Objective:To investigate the clinical effecty of budesonide(BUD)for inhalation combined with oral montelukast sodium(MON)and levocetirizine(LCT)in the treatment of cough variant asthma(CVA)in children.Methods:A total of 100 children with cough variant asthma admitted to the First Affiliated Hospital of Jinzhou Medical University from January 2019 to December 2021 were selected as the study subjects.They were randomly divided into observation group and control group,with 50 cases in each group.The control group was treated with BUD inhalation aerosol and the observation group was treated with MON and LCT on the basis of the control group.The clinical efficacy,levels of serum inflammatory factors,lung function index,and occurrence of adverse reactions in the two groups were compared.Results:The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.001).After treatment,the levels of serum immunoglobulin E,interleukin-4 and interleukin-6 in the two groups were lower than those before treatment,and the observation group was lower than the control group,and the difference was statistically significant(P<0.001).After treatment,the forced expiratory volume in 1 second,forced vital capacity,ratio of forced expiratory volume in 1 second to forced vital capacity,and arterial oxygen saturation in the two groups were higher than those before treatment,and the observation group was higher than the control group,and the differences were statistically significant(P<0.001).The incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.029).Conclusion:BUD for inhalation combined with oral MON and LCT in the treatment of pediatric CVA has a significant effect,it can reduce the levels of inflammatory factors,improve lung function and arterial oxygen saturation,and the incidence of adverse reactions is low,the safety is high,so it is worthy of clinical application.
