舌骨位置与颅底凹陷相关性的X线影像学研究

Correlation between the position of hyoid bone and basilar invagination in radiological X-ray imaging

目的:探讨舌骨位置与齿状突位置在诊断枕颈交界区(Occipitocervical junction,OCJ)异常中的相关性。方法:随机回顾性研究了204例患者(女132例,男72例)的颈椎X线侧位片。采用Redlund-Johnell线获得齿状突位置,以舌骨前下缘到C3前下缘的距离为舌骨位置。为了评估舌骨位置与齿状突的相互关系,进行了相关性分析。结果:总组齿状突位置与舌骨位置存在中度相关,具有统计学意义(r=-0.456,P<0.01)。线性回归方程为AB=0.371CD+24.665,其中AB=C_(2)椎体下终板中心至McGregor线的最短距离,CD=C_(3)前下缘至舌骨前下缘的距离。年龄与齿状突位置呈弱负相关(r=-0.266,P<0.01),与舌骨位置无相关性(P>0.05)。男性齿状突位置和舌骨位置之间没有相关性,而女性这两个值呈弱相关(r=0.298,P<0.05)。结论:研究表明,舌骨位置和齿状突尖端之间存在一定程度的相互关系,即AB=0.371CD+24.665(r=0.44,P<0.01),因而可进一步评估OCJ。此外,年龄与性别有一定影响。

Objective:To investigate the correlation between the position of the hyoid bone and the odontoid process in the diagnosis of occipitocervical junction(OCJ)abnormalities.Methods:A randomized retrospective study of 204 patients(132 females and 72 males)with cervical spine lateral radiographys was conducted.The location of the odontoid was obtained using the Redlund-Johnell line,and the location of the hyoid was determined by the distance from the anterior inferior margin of the hyoid to the anterior inferior margin of C_(3).Correlation analysis was performed to evaluate the relationship between hyoid position and the odontoid process.Results:There was a moderate correlation between the position of the odontoid and that of the hyoid in the total group(r=-0.456,P<0.01).The linear regression equation was AB=0.371CD+24.665,where AB is the shortest distance from the center of the inferior endplate of C_(2) vertebral body to the McGregor line,and CD is the distance from the anterior inferior margin of C_(3) to the anterior inferior margin of the hyoid bone.There was a weak negative correlation between age and the position of the odontoid(r=-0.266,P<0.01)and had no correlation with the hyoid position(P>0.05).There was no correlation between the position of the odontoid and the position of the hyoid in males,while there was a weak correlation between the two values in females(r=0.298,P<0.05).Conclusion:There is a correlation between the position of the hyoid bone and the tip of odontoid,i.e.AB=0.371CD+24.665(r=0.44,P<0.01),to further evaluate OCJ.In addition,age and sex play a role.
