以女性生殖系统病变为首发症状的14例淋巴瘤的超声表现

Ultrasonic findings of primary lymphoma of the female reproductive system in 14 cases

目的:分析以女性生殖系统病变为首发症状的淋巴瘤的超声表现(二维超声和彩色多普勒超声),探讨超声对该病的诊断价值。方法:回顾性分析2013年9月—2021年12月我院经手术病理及免疫组织化学染色确诊的14例女性生殖系统淋巴瘤患者的超声资料,并对其图像特点进行分析总结。结果:14例患者共检出21个病灶,9例为单发病灶,5例为多发病灶。二维超声表现:21个病灶中有18个表现为盆腔内实性低回声,2个表现为无回声,1个表现为不均质稍强回声;13个病灶边界清晰,8个病灶边界欠清。彩色多普勒超声显示21个病灶内部均探及血流信号,其中17个病灶探及较丰富血流信号;5个病灶测量血流频谱,呈中等阻力型(RI 0.54~0.78)。结论:女性生殖系统淋巴瘤超声特点大多表现为附件区、子宫或宫颈管内实性低或弱回声团块,团块体积相对较大、形态较规则、边界较清晰、内部可探及较丰富的血流信号等,超声医生检查时如果遇到病变出现上述征象应考虑淋巴瘤的可能性。

Objective:To investigate the ultrasonic manifestations(two-dimensional ultrasonography and color Doppler ultrasonography)of primary lymphoma of the female reproductive system,and to explore the diagnostic value of ultrasound.Methods:From September 2013 to December 2021,the ultrasonic data of 14 cases with primary lymphoma in female reproductive system confirmed by surgical and immunohistochemical pathology were retrospectively analyzed and summarized.Result:A total of 21 lesions were detected in 14 cases,including nine with single lesion and five with multiple lesions.In two-dimensional ultrasound,it was showed that 18 of the 21 lesions with solid hypoechogenicity,two with anechogenicity,and one with heterogeneous slightly hyperechogenicity in the pelvic cavity.The boundary of 13 lesions was clear,and the boundary of eight lesions was unclear.In color Doppler ultrasound,it was showed that 17 lesions with abundant blood flow signals.In spectral Doppler ultrasound,it was showed that five lesions with moderate blood flow resistance(RI 0.54~0.78).Conclusion:The ultrasonic features of primary lymphoma of the female reproductive system were low or weak solid echo masses in the attachment area,uterus and the cervical canal,which had relatively large volume,regular morphology,the clear boundary,abundant blood flow signals.If the ultrasonic doctor checks the lesion,the possibility of lymphoma should be considered if the above signs occur.
