基于DRG的精细化管理措施对NC19组医疗服务效率的影响

The Influence of DRG-based Lean Management Measures on the Efficiency of Medical Services in NC19 Group

目的探讨基于疾病诊断相关分组的精细化管理对某院NC19组子宫(除子宫腔内病变以外)手术医疗服务效率的影响,为医院DRG精细化管理提供参考依据。方法以2021年3月15日-2022年3月15日优化DRG精细化管理措施前的164份NC19组病例作为对照组,以2022年3月15日-2023年3月15日优化DRG精细化管理措施后的191份NC19组病例作为观察组。比较精细化管理优化措施前后对照组和观察组的住院服务效率指标变化。结果两组患者的基本特征比较,差异无统计学意义,P>0.05。对照组平均住院日中位数9.5天,观察组为8天,两组比较P<0.05,差异具有统计学意义。对照组次均费用中位数为20571.41元,观察组为18683.12元,两组比较差异具有统计学意义,P<0.05。结论基于DRG付费方式的精细化管理措施有助于提高NC19组的医疗服务效率。

Objectives This study aims to explore the effect of lean management based on disease diagnosis-related grouping(DRG)on the efficiency of uterine surgery(except intrauterine lesions)in the NC19 group of a hospital,so as to provide a reference for hospital DRG lean management.Methods 164 cases of NC19 group before optimizing DRG refined management measures from March 15,2021 to March 15,2022were used as the control group.191 cases of NC19 group after optimizing DRG refined management measures from March 15,2022 to March 15,2023 were used as the observation group.The improvement degrees of medical service efficiency indexes of NC19 group were compared between before and after the implementation of lean management.Results There was no statistically significant difference in average age and comorbidity between two groups(P>0.05).The median length of stay in the control group was 9.5 days,and that of the observation group was 8 days.The comparison between the two groups was P<0.05,and the difference was statistically significant.The median cost per visit in the control group was 20,571.41 yuan,and that in the observation group was 18,683.12 yuan.The difference between the two groups was statistically significant(P<0.05).Conclusions Refined management measures based on DRG payment methods can help improve the efficiency of medical services in the NC19 group.
