下肢气压仪预防宫外孕失血性休克患者下肢深静脉血栓的效果及其对凝血指标的影响

Effect of lower extremity barograph on prevention of lower extremity deep vein thrombosis in patients with hemorrhagic shock during ectopic pregnancy and its influence on coagulation index

目的:探讨下肢气压仪预防宫外孕失血性休克(Hemorrhagic shock,HS)患者下肢深静脉血栓(Deep venous thrombosis,DVT)的效果及其对凝血指标的影响。方法:选取2021年1月至2022年6月期间我科收治的80例宫外孕HS患者作为研究对象,按随机数字表法分为护理组和气压仪组,各40例。护理组采取集束化护理,气压仪组在护理组基础上增加下肢气压仪干预。对比两组患者DVT发生率、凝血指标、下肢静脉血流速度。结果:气压仪组、护理组发生DVT的例数分别为1例、8例。气压仪组的DVT发生率显著低于护理组(χ^(2)=4.507,P=0.034)。干预2 d、7 d后,气压仪组部分凝血活酶时间(Activated partial thromboplastin time,APTT)、凝血酶原时间(Prothrombin time,PT)均长于护理组,D二聚体(D dimer,D-D)水平低于护理组(P<0.05);气压仪组的股总静脉、股深静脉、腘静脉的血流速度均大于护理组(P<0.05)。结论:下肢气压仪能显著加快宫外孕HS患者的下肢血液循环,并能改善凝血指标,有效预防术后DVT的发生。

Objective:To investigate the effect of Hemorrhagic shock(HS)prevention of Deep venous thrombosis(DVT)with lower extremity barograph and its effect on coagulation indexes.Methods:A total of 80 patients with ectopic pregnancy HS from January 2021 to June 2022 in our department were selected as research objects and divided into nursing group and barometric group according to random number table method.In the nursing group,40 cases received cluster nursing,and in the barometric group,40 cases received lower extremity barometric intervention.The incidence of DVT,coagulation index and lower limb venous blood flow velocity were compared between the two groups.Results:The cases of DVT in barometric group and nursing group were 1 and 8,respectively.The incidence of DVT in barometric group was significantly lower than that in nursing group(c2=4.507,P=0.034).After 2 d and 7 d intervention,the Activated partial thromboplastin time(APTT)and Prothrombin time(PT)in the air pressure group were all longer than those in the nursing group,The level of D-D was lower than that of nursing group(P<0.05);The blood flow velocity of common femoral vein,deep femoral vein and popliteal vein in barometric group was higher than that in nursing group(P<0.05).Conclusion:Lower extremity baronometer can significantly accelerate lower extremity blood circulation in ectopic pregnancy HS patients,improve coagulation index,and effectively prevent the occurrence of postoperative DVT.
