NLRP3炎症小体在急性胰腺炎中的作用及中药干预研究进展

Role of NLRP3 Inflammasome in Acute Pancreatitis and Intervention of Traditional Chinese Medicine:A Review

急性胰腺炎(AP)是临床上常见的急性腹部疾病,但是目前对于其的发病机制仍存在争议,以往研究证实炎症反应贯穿AP始终,且与其他发病机制具有一定的相关性。随着对AP炎症反应的研究不断深入,NLRP3炎性小体被证实参与多种病理条件下的炎症反应。NLRP3炎症小体主要由DAMP受体NLRP3、衔接分子ASC和Caspase-1组成,NLRP 3炎症小体能够促进IL-1β、IL-6的成熟,这两种炎症因子与AP密切相关,并且与疾病的严重程度呈正相关。目前针对胰腺炎尚无特异性药物治疗,且抗感染药物的应用存在一定争议,中药依靠辨证论治,整体观念的特性在急性胰腺炎的治疗过程中发挥着重要作用。本文通过探讨NLRP3炎症小体与AP的关系,挖掘其在AP中可能存在的调控机制及作用靶点,总结中医药通过调控NLRP3炎症小体,减少炎症细胞因子释放,对AP发挥抗炎作用的研究进展,以期为AP的防治提供新的思路。

Acute pancreatitis(AP) is a common acute abdominal disease in clinical practice,but its pathogenesis is still controversial.Previous studies have confirmed that inflammatory reaction runs through AP and has a certain correlation with other pathogenesis.With in-depth research on the inflammatory response of AP,NLRP3 inflammasome has been confirmed to participate in the inflammatory responses under various pathological conditions.NLRP3 inflammasome,mainly composed of DAMP receptor NLRP3,adapter molecule ASC,and caspase-1,can promote the maturation of IL-1β and TL-6,and these two inflammatory factors are closely correlated with AP,and positively correlated with the severity of the disease.At present,there is no specific drug treatment for pancreatitis,and the application of anti-infective drugs is controversial.The treatment of traditional Chinese medicine(TCM) is based on syndrome differentiation,and the characteristics of holistic view play an important role in the treatment of AP.This article discussed the relationship between NLRP3 inflammasome and AP,explored the underlying regulatory mechanism and targets in AP,and reviewed the anti-inflammatory effect of TCM on AP by regulating NLRP3 inflammasome and reducing the release of inflammatory cytokines,aiming at providing new ideas for the prevention and treatment of AP.
