玉环市农村社区成人女性2型糖尿病与糖调节受损的年龄差异

The age-related sex differences on T2DM and IGR of adults in rural communities of Yuhuan County

目的了解浙江省玉环市农村社区成人女性血糖异常现况,分析成人女性2型糖尿病(Type 2 diabetes mellitus,T2DM)和糖调节受损(Impaired glucose regulation,IGR)的年龄差异。方法采用横断面研究设计,于2012年6—12月在浙江省玉环市随机整群抽取的3个乡镇面对面调查20岁及以上户籍女性居民15838人,同时进行体检和血生化检查。按照第6次全国人口普查结果进行患病率标化。结果玉环市农村社区成人女性T2DM和IGR粗患病率分别为13%和18%,标准化患病率分别为8.4%和13.2%,糖尿病知晓率仅为54.5%。空腹血糖及平均糖负荷后2 h血糖随年龄增高均呈上升趋势(χ^(2)_(趋势)=2473.85,P<0.001;χ^(2)_(趋势)=3573.15,P<0.001);IGR、T2DM患病率随着年龄的增加而增加,至60岁保持较高的水平(χ^(2)_(IGR趋势)=25.15,P<0.001;χ^(2)_(DM趋势)=38.31,P<0.001),其中IGR的患病率稳定上升,而T2DM的患病率在40岁以后快速上升,60岁之后增长速度略微减缓。结论玉环市农村社区成人女性2型糖尿病和IGR患病水平均较高,知晓率较低,其发病率与年龄密切相关。

Objective To understand the status of abnormal blood glucose among adult women in rural communities of Yuhuan city,Zhejiang Province,and to analyze the age difference of Type 2 diabetes mellitus(T2DM)and Impaired glucose regulation(IGR)。Methods A cross-sectional study design was adopted.A total of 15,838 female residents aged 20 or above were randomly selected from three townships in Yuhuan City,Zhejiang Province from June to December 2012.Physical examination and blood biochemical examination(including blood glucose)were conducted at the same time.Prevalence was standardized according to the results of the sixth National Census.Results The crude prevalence rates of T2DM and ICR were 13%and 18%,and the standardized prevalence rates were 8.4%and 13.2%,respectively.The awareness rate of diabetes was only 54.5%.Fasting blood glucose(FBG)and mean 2 h post-glucose load blood glucose(FBG)showed an increasing trend with age(χ_(trend)^(2)=2473.85,P<0.001;χ_(trend)^(2)=3573.15,P<0.001);The prevalence of ICR and T2DM increased with age and remained at a high level until 60 years old(χ_(IGR trend)^(2)=25.15,P<0.001;χ_(DM trend)=38.31,P<0.001),The prevalence of IGR increased steadily,while the prevalence of T2DM increased rapidly after the age of 40 and slowed down slightly after the age of 60.Conclusion The level of type 2 diabetes and IGR among adult women in rural communities of Yuhuan City was high,and the awareness rate was low,and the incidence was closely related to age.
