临床护理路径引导扁平化责任制护理对胫骨平台骨折患者术后恢复的影响

Effect of flat responsibility nursing guided by clinical nursing path on postoperative recovery of tibial plateau fracture

目的:探讨临床护理路径引导扁平化责任制护理对胫骨平台骨折患者术后恢复的影响。方法:选取聊城市中医医院2018年11月至2021年6月期间接诊的116例需行胫骨平台骨折术患者为研究对象,根据不同的护理方法,分为研究组62例和对照组54例。对照组患者给予常规护理,研究组患者给予临床护理路径引导扁平化责任制护理模式。术后6个月,观察并比较两组患者膝关节功能恢复情况、骨折愈合时间、撤拐时间、患肢负重时间、并发症发生率及护理满意度。结果:研究组患者护理后的疼痛(25.46±6.94)分、稳定性(7.23±0.28)分、关节活动度(17.23±0.56)分、肌力(8.02±0.54)分、功能(17.03±1.02)分、屈膝畸形评分(8.23±1.23)分均显著高于对照组的(13.28±5.68)分、(5.02±0.36)分、(12.64±0.62)分、(5.99±0.58)分、(13.06±1.12)分、(6.32±0.84)分,差异均有统计学意义(均P<0.05);研究组患者术后骨折愈合时间、撤拐时间、患肢负重时间均明显短于对照组,差异有统计学意义(P<0.05);研究组患者的护理满意度为96.77%(60/62),明显高于对照组的72.22%(39/54),差异有统计学意义(P<0.05);研究组患者并发症发生率为3.23%(2/62),显著低于对照组24.07%(11/54),差异有统计学意义(P<0.05)。结论:对胫骨平台骨折患者术后采用临床护理路径引导扁平化责任制护理,有助于改善患者术后膝关节功能,能有效缩短/降低患者骨折愈合时间、撤拐时间、患肢负重时间、并发症发生率,并能提高患者护理满意度。

Objective To explore the effect of clinical nursing path guided flat responsibility system on postoperative recovery of patients with tibial plateau fracture.Methods A total of 116 patients requiring tibial plateau fracture treated in Liaocheng Hospital of Traditional Chinese Medicine from November 2018 to June 2021 were selected as the study objects.According to different nursing methods,they were divided into the study group(62 cases)and the control group(54 cases).Patients in the control group were given routine nursing,while patients in the study group were given clinical nursing path guidance flat responsibility system nursing model.Six months after operation,the functional recovery of knee joint,fracture healing time,removal time,weight-bearing time of affected limb,incidence of complications and nursing satisfaction of the two groups were observed and compared.Results The pain score of the study group after nursing was(25.46±6.94)points,stability score was(7.23±0.28)points,joint motion score was(17.23±0.56)points,muscle strength score was(8.02±0.54)points,function score was(17.03±1.02)points,and bending knee deformity score was(8.23±1.23)points,which were significantly higher than(13.28±5.68)points,(5.02±0.36)points,(12.64±0.62)points,(5.99±0.58)points,(13.06±1.12)points and(6.32±0.84)points of the control group,and the differences were statistically significant(P<0.05).The postoperative fracture healing time,abduction time and weight-bearing time of the affected limb in the study group were significantly shorter than those in the control group,and the differences were statistically significant(P<0.05).The nursing satisfaction of patients in the study group was 96.77%(60/62),which was significantly higher than 72.22%(39/54)in the control group,and the difference was statistically significant(P<0.05).The complication rate of the study group was 3.23%(2/62),which was significantly lower than that of the control groups〔24.07%(11/54)〕,and the difference was statistically significant(P<0.05).Conclusions The flat responsibility nursing guided by clinical nursing path in the operation of tibial plateau fracture is helpful to improve the recovery of knee function after operation,effectively shorten the fracture healing time,abduction time,weight-bearing time of affected limb and the incidence of complications,and improve the nursing satisfaction of patients.
