糖调节受损并良性前列腺增生患者血清瘦素、胰岛素抵抗与前列腺体积的关系研究 被引量:1

摘要 目的探讨糖调节受损并良性前列腺增生(BPH)患者血清瘦素(Lep)、胰岛素抵抗与前列腺体积(PV)的关系。方法纳入2020年2月至2021年12月我院收治的150例BPH患者作为BPH组,依据国际糖尿病联盟报告,根据OGTT结果分为糖耐量正常组(NGT,n=72例)、空腹血糖受损组(IFG,n=30例)、糖尿病减低组(IGT,n=48例)。同期选取150例健康体检者作为健康组,测定两组研究对象人体参数(身高、体重、腰围、臀围),采集空腹静脉血测定空腹血糖、胰岛素、瘦素、血脂、前列腺特异抗原(PSA)及计算胰岛素抵抗指数(HOMA-IR),以彩超测定PV,采用Pearson相关分析探讨糖调节受损并BPH患者血清Lep、胰岛素抵抗与PV的相关性。结果BPH组BMI、腰臀比高于健康组(P<0.05);BPH组内,NGT组、IFG组、IGT组BMI、腰臀比对比(P<0.05)。BPH组Lep、FINS、FPG、HOMA-IR、PSA、TG、TC、LDL-C高于健康组,HDL-C低于健康组(P<0.05);BPH组内,NGT组、IFG组、IGT组血液标本检测结果对比(P<0.05)。BPH组PV高于健康组(P<0.05);BPH组内,NGT组、IFG组、IGT组PV对比(P<0.05)。经Pearson相关分析,PV与Lep、HOMA-IR、PSA、TG呈正相关(r=0.653、0.685、0.423、0.595,P<0.05),与HDL-C呈负相关(r=-0.632,P<0.05)。结论糖调节受损并BPH组患者血清Lep、胰岛素抵抗与PV有一定相关性,临床可加强监测辅助疾病诊疗。 Objective To investigate the relationship between serum leptin(Lep),insulin resistance and prostate volume(PV)in patients with impaired glucose regulation and benign prostatic hyperplasia(BPH).Methods 150 patients with BPH admitted to our hospital from February,2020 to December,2021 were included as BPH group.According to the report of International Diabetes Federation,these patients were divided into the normal glucose tolerance group(NGT,n=72 cases),impaired fasting glucose group(IFG,n=30 cases)and reduced diabetes group(IGT,n=48 cases)according to OGTT results.During the same period,150 healthy subjects were selected as the healthy group.Human parameters(height,weight,waist circumference and hip circumference)of the subjects in the two groups were measured.Fasting venous blood was collected to measure fasting blood glucose,insulin,leptin,blood lipid,prostate-specific antigen(PSA)and insulin resistance index(HOMA-IR)were calculated.Pearson correlation analysis was used to investigate the correlation between serum Lep,insulin resistance and PV in patients with impaired glucose regulation and BPH.Results BMI and waist-to-hip ratio in BPH group were higher than those in the healthy group(P<0.05).In BPH group,BMI and waist-to-hip ratio in NGT group,IFG group and IGT group were compared(P<0.05).Lep,FINS,FPG,HOMA-IR,PSA,TG,TC and LDL-C in BPH group were higher than those in the healthy group,while HDL-C was lower than those in the healthy group(P<0.05).Comparison of blood samples in BPH group showed that NGT group,IFG group and IGT group were significantly different(P<0.05).PV in BPH group was higher than that in healthy group(P<0.05).In BPH group,the PV of NGT group,IFG group and IGT group was compared and significantly different(P<0.05).According to Pearson correlation analysis,PV was positively correlated with Lep,HOMA-IR,PSA and TG(r=0.653,0.685,0.423,0.595,P<0.05),while negatively correlated with HDL-C(r=-0.632,P<0.05).Conclusion Serum Lep,insulin resistance and PV in patients with impaired glucose regulation and BPH group are correlated to a certain extent.Clinical monitoring should be strengthened to assist disease diagnosis and treatment.
