船舶油漆暴露对男性肺癌患病风险的影响

摘要 目的探讨船舶油漆暴露对男性肺癌患病风险的影响,为肺癌防治提供科学依据。方法采用1∶1配对病例对照研究方法,选取2017年1月至2022年6月中国人民解放军联勤保障部队第九六七医院收治的经病理学确诊、新诊断的原发性男性肺癌患者作为病例组,按照病例组性别、年龄(±1岁)分布在同一时间、同一医院随机选取非肺癌住院患者作为对照组,根据标准化调查问卷对研究对象进行面对面访谈,采用条件Logistic回归模型进行数据分析。结果共纳入433例男性肺癌病例、433例男性医院对照,在调整体质指数、吸烟史、既往呼吸系统疾病史、恶性肿瘤家族史后,多因素条件Logistic回归分析显示,暴露于油漆、船舶油漆的研究对象患肺癌风险分别是非暴露者的1.835倍和2.278倍[油漆:OR值(95%CI)=1.835(1.129,2.981);船舶油漆OR值(95%CI)=2.278(1.102,4.712)],同时随着暴露时间的延长,两者患肺癌的风险均逐渐升高[油漆暴露时间1~10年OR值(95%CI)=1.181(0.587,2.377),>10年OR值(95%CI)=2.687(1.357,5.324);船舶油漆暴露时间1~10年OR值(95%CI)=1.547(0.678,3.530,>10年OR值(95%CI)=8.982(1.136,71.011)]。结论船舶油漆暴露是男性肺癌发生发展的独立危险因素。相比于油漆暴露,暴露于船舶油漆的男性患肺癌的风险更高。随着船舶油漆暴露时间的延长,男性患肺癌的风险逐渐升高。 Objective To explore the effects of marine paint exposure on lung cancer risk in male,so as to provide a scientific basis for the prevention and treatment of lung cancer.Methods A 1∶1 matched case-control study method was used to select pathologically confirmed and newly diagnosed primary male lung cancer patients admitted to the 967th Hospital of Joint Logistic Support Force of PLA from January 2017 to June 2022 as the case group,non-lung cancer inpatients were randomly selected as the control group in the same hospital at the same time according to the gender and age of the cases(±1years).The subjects were interviewed face-to-face according to standardized questionnaires,and the data were analyzed by multivariable conditional Logistic regression models.Results A total of 433 male lung cancer cases and 433 male hospital control cases were collected.After adjusting for smoking habit,previous respiratory disease history,body mass index,family history of malignant tumor,multivariate Logistic regression analysis showed that the risk of lung cancer in subjects exposed to paint and marine paint was 1.835 times and 2.278 times that of non-exposed subjects,respectively[paint OR(95%CI)=1.835(1.129,2.981),marine paint OR(95%CI)=2.278(1.102,4.712)].At the same time,it was also found that with the duration of exposure time,the risk of lung cancer in both increased gradually[(paint 1~10 years OR(95%CI)=1.181(0.587,2.377),>10 years OR(95%CI)=2.687(1.357,5.324);marine paint 1~10 years OR(95%CI)=1.547(0.678,3.530),>10 years OR(95%CI)=8.982(1.136,71.011)].Conclusion Marine paint is an independent risk factor for the development of lung cancer in males.Compared with paint exposure,the risk of lung cancer is higher.At the same time,the longer the exposure time,the higher the risk of lung cancer in males.
