基于股骨近端影像学参数预测PFNA术后失效风险的研究进展

Research Progress in Predicting the Risk of Nonunion after PFNA Based on Radiographic Parameters of the Femur

随着我国社会人口老龄化程度逐渐加深,股骨转子间骨折患者也日益增多,尽快手术干预已成为股骨转子间骨折的诊治共识。PFNA治疗股骨转子间骨折在力学原理和抗旋转功能上具有显著优势,是目前应用最广的髓内固定方式。然而,随着手术量的持续增加,内固定失败的病例数也随之上升,尽管许多研究探讨了防旋股骨髓内钉(PFNA)内固定失败的原因,但其中仅有少数单独针对于影像学资料来进行研究。本文系统地总结了股骨近端的影像学参数及PFNA术后的失效风险,目标是早期识别和预防内固定失效,及时避免失效的发生,为临床医生预防内固定的失效提供参考。

With the aging of Chinas population,there are an increasing number of patients with intertrochanteric fractures,and surgical intervention as soon as feasible has become the diagnosis and treatment consensus for intertrochanteric fractures.PFNA has significant advantages in mechanical principles and anti-rotation function in the treatment of intertrochanteric fractures,and it is currently the most popularly used fixation method.However,as the number of operations continues to rise,the incidence of internal fixation failure rises as well.Although many studies investigate the causes of internal fixation failure with femoral nail antirotation(PFNA),only a few of them concentrate solely on imaging data.As a result,this article systematically summarizes the imaging parameters of the proximal femur and the risk of failure after PFNA,with the goal of identifying and preventing internal fixation failure early,avoiding failure in time,and providing clinicians with a reference to prevent internal fixation failure.
