慢性心力衰竭患者生活质量与自我护理能力及家庭支持程度的相关性研究

Relationship between quality of life and self-care ability and family support in patients with chronic heart failure

目的探讨慢性心力衰竭(chronic heart failure,CHF)患者自我护理能力及家庭支持程度对其生活质量的影响。方法选取2019年1月至2022年1月杭州市第九人民医院收治的213例CHF患者为研究对象,根据明尼苏达心力衰竭生存质量问卷(Minnesota living with heart failure questionnaire,MLHFQ)评分将其分为良好组(MLHFQ≥120分,81例)与不良组(MLHFQ<120分,132例),比较两组患者的一般临床资料、MLHFQ评分、心力衰竭自我护理指数(self-care of heart failure index,SCHFI)量表和家庭支持自评量表(perceived social support from family,PSS-Fa)评分,采用多因素Logistic回归分析探讨影响患者生活质量的相关因素。结果两组患者的文化程度、心功能分级及合并基础疾病数比较,差异均有统计学意义(P<0.05);良好组患者的MLHFQ、SCHFI、PSS-Fa评分均显著高于不良组(P<0.05);多因素Logistic回归分析显示,SCHFI评分降低、PSS-Fa评分降低、文化程度低、心功能分级高及合并基础疾病多均是影响CHF患者生活质量的独立危险因素(P<0.05)。结论CHF患者的家庭支持程度、自我护理能力和生活质量水平均有待提升,加强CHF患者的自我护理能力和家庭支持程度,有利于提高其生活质量。

Objective To investigate the effects of self-care ability and family support on quality of life of patients with chronic heart failure(CHF).Methods A total of 213 CHF patients admitted to Hangzhou Ninth People’s Hospital from January 2019 to January 2022 were selected as the study subjects.According to the Minnesota living with heart failure questionnaire(MLHFQ)score,the patients were divided into good group(81 cases with MLHFQ≥120 points)and poor group(132 cases with MLHFQ<120 points).General clinical data,MLHFQ score,self-care of heart failure index(SCHFI)scale and perceived social support from family(PSS-Fa)score were compared between the two groups.Multivariate Logistic regression analysis was used to explore the related factors affecting the quality of life of patients.Results There were statistically significant differences in educational level,cardiac function grade and number of combined underlying diseases between the two groups(P<0.05).The scores of MLHFQ,SCHFI and PSS-Fa in good group were significantly higher than those in poor group(P<0.05).Multivariate Logistic regression analysis showed that lower SCHFI score,lower PSS-Fa score,lower educational level,higher cardiac function grade and more common underlying diseases were independent risk factors affecting the quality of life of CHF patients(P<0.05).Conclusion The level of family support,self-care ability and quality of life of CHF patients need to be improved.Strengthening self-care ability and family support of CHF patients is conducive to improving their quality of life.
