袋鼠式护理联合保护性睡眠干预在早产儿高胆红素血症中的应用

Application of kangaroo nursing combined with protective sleep intervention in hyperbilirubinemia of premature infants

目的探究袋鼠式护理联合保护性睡眠干预在早产儿高胆红素血症中的应用价值。方法选取2020年1月至2022年7月在浙江大学医学院附属第二医院临平院区出生的高胆红素血症早产儿80例,根据护理方式不同,将患儿分为对照组和观察组,每组各40例。对照组患儿给予常规护理干预,观察组患儿给予袋鼠式护理联合保护性睡眠干预,比较两组患儿的临床效果、总胆红素水平、睡眠质量及并发症发生率。结果观察组患儿的哭闹时间、高胆红素血症消退时间、首次排便时间、胎便转黄时间及住院时间均显著短于对照组,安静睡眠时间显著长于对照组(P<0.05);干预后,两组患儿的总胆红素水平均随时间延长而降低(P<0.05),干预后3d、5d、7d,观察组患儿的总胆红素水平均显著低于对照组(P<0.05);观察组患儿的睡眠优良率显著高于对照组(95.00%vs.80.00%,χ^(2)=4.114,P=0.042),总并发症发生率显著低于对照组(χ^(2)=4.242,P=0.039)。结论袋鼠式护理联合保护性睡眠干预不仅能缩短患儿的康复时间、延长安静睡眠时间、提高睡眠质量,还能降低并发症发生率,家属认可度高。

Objective To explore the application value of kangaroo nursing combined with protective sleep intervention in premature infants with hyperbilirubinemia.Methods A total of 80 premature infants with hyperbilirubinemia born in Linping District,the Second Affiliated Hospital,Zhejiang University School of Medicine from January 2020 to July 2022 were selected.According to different nursing methods,the infants were divided into control group and observation group,40 cases in each group.The control group was given routine nursing intervention,the observation group was given kangaroo nursing combined with protective sleep intervention.The clinical effect,total bilirubin level,sleep quality and complication rate of the two groups were compared.Results The crying time,resolution time of hyperbilirubinemia,first defecation time,yellow meconium turning time and hospital stay in observation group were significantly shorter than those in control group,and quiet sleep time was significantly longer than that in control group(P<0.05).After intervention,the level of total bilirubin in both groups decreased with the extension of time(P<0.05),the level of total bilirubin in observation group was significantly lower than that in control group at 3d,5d and 7d after intervention(P<0.05).The sleep good rate in observation group was significantly higher than that in control group(95.00%vs.80.00%,χ^(2)=4.114,P=0.042),and the total complication rate was significantly lower than that in control group(χ^(2)=4.242,P=0.039).Conclusion Kangaroo nursing combined with protective sleep intervention can not only shorten the recovery time of infants,extend the quiet sleep time,improve the sleep quality,but also reduce the incidence of complications,which is highly recognized by family members.
