Significance of brachial-ankle pulse wave velocity in hemodialysis patients

目的分析肱踝脉搏波传导速度(baPWV)在血液透析患者中的意义。方法选取2016年3月至2018年3月在我院血透中心长期规律行血液透析的228例患者作为研究对象,将baPWV高于平均值的患者纳入高baPWV组(101例),低于平均值的患者纳入低baPWV组(127例)。分析baPWV与临床相关指标的关系;分析影响血液透析患者预后的因素。结果高baPWV组的年龄、透析前收缩压、磷、全段甲状旁腺激素(iPTH)、总胆固醇水平高于低baPWV组,白蛋白水平低于低baPWV组,差异具有统计学意义(P<0.05)。多因素Cox回归分析显示,年龄>60岁、透析前收缩压>141.25 mmHg、baPWV>23.17 m/s是影响血液透析患者预后的独立危险因素(P<0.05)。结论高baPWV与血液透析患者的多种临床指标相关,且是预测患者预后的独立危险因素,具有重要的临床意义。

Objective To analyze the significance of brachial-ankle pulse wave velocity(baPWV)in hemodialysis patients.Methods A total of 228 patients who underwent long-term regular hemodialysis in the hemodialysis center of our hospital from March 2016 to March 2018 were selected as the research objects.Patients with above-average baPWV were included in high baPWV group(101 cases),and patients with below-average baPWV were included in low baPWV group(127 cases).The relationship between baPWV and clinical related indicators were analyzed;the factors affecting the prognosis of hemodialysis patients were analyzed.Results The age,pre-dialysis systolic blood pressure,phosphorus,intact parathyroid hormone(iPTH)and total cholesterol levels in the high baPWV group were higher than those in the low baPWV group,the albumin level was lower than that in the low baPWV group,and the differences were statistically significant(P<0.05).Multivariate Cox regression analysis showed that age>60 years old,pre-dialysis systolic blood pressure>141.25 mmHg and baPWV>23.17 m/s were independent risk factors affecting the prognosis of hemodialysis patients(P<0.05).Conclusion High baPWV is associated with a variety of clinical indicators in hemodialysis patients,and is an independent risk factor for predicting the prognosis of patients,which has important clinical significance.
