经尿道前列腺钬激光剜除术治疗大体积前列腺增生的效果及对HSP27、VEGF、bFGF水平的影响

Effect of transurethral holmium laser enucleation of the prostate in the treatment of large-volume benign prostatic hyperplasia and its influences on HSP27,VEGF and bFGF levels

目的探究经尿道前列腺钬激光剜除术治疗大体积前列腺增生的效果。方法按照随机数字表法将2019年2月至2021年3月收治的80例大体积前列腺增生患者分为对照组(40例,经尿道前列腺电切术)与观察组(40例,经尿道前列腺钬激光剜除术)。比较两组的治疗效果。结果术后3个月,观察组的热休克蛋白27(HSP27)、血管内皮生长因子(VEGF)、碱性成纤维细胞生长因子(bFGF)水平低于对照组(P<0.05)。术后3个月,观察组的国际勃起功能指数-5(IIEF-5)评分高于对照组,国际前列腺症状评分表(IPSS)、生活质量评分量表(QOL)评分低于对照组(P<0.05)。观察组的术后并发症总发生率低于对照组(P<0.05)。结论经尿道前列腺钬激光剜除术治疗大体积前列腺增生的效果更好,不仅能调节HSP27、VEGF及bFGF水平,改善前列腺症状,有效提高患者勃起功能及生活质量,还能减少并发症。

Objective To explore the effect of transurethral holmium laser enucleation of the prostate in the treatment of large-volume benign prostatic hyperplasia.Methods A total of 80 patients with large-volume benign prostatic hyperplasia admitted from February 2019 to March 2021 were divided into control group(40 cases,transurethral resection of the prostate)and observation group(40 cases,transurethral holmium laser enucleation of the prostate)according to the random number table method.The therapeutic effects of the two groups were compared.Results At 3 months after operation,the levels of heat shock protein 27(HSP27),vascular endothlial growth factor(VEGF)and basic fibroblast growth factor(bFGF)in the observation group were lower than those in the control group(P<0.05).At 3 months after operation,the International Index of Erectile Function-5(IIEF-5)score of the observation group was higher than that of the control group,the International Prostate Symptom Score(IPSS)and Quality of Life(QOL)scores were lower than those of the control group(P<0.05).The total incidence of postoperative complications in the observation group was lower than that in the control group(P<0.05).Conclusion Transurethral holmium laser enucleation of the prostate is more effective in the treatment of large-volume benign prostatic hyperplasia,it can not only regulate the levels of HSP27,VEGF and bFGF,improve the symptoms of the prostate,effectively improve the erectile function and quality of life of the patients,but also reduce complications.
