多媒体教育联合临床路径情绪引导对消化内镜检查患者应激反应、配合度及检查结局的影响

Effects of multimedia education combined with clinical pathway emotional guidance on stress response,cooperation degree and examination outcome of patients undergoing digestive endoscopy

摘要 目的探讨多媒体教育联合临床路径情绪引导对消化内镜检查患者应激反应、配合度及检查结局的影响。方法选取我院2020年2月至2022年2月接受消化内镜检查的230例患者为研究对象,按照1∶1的模式将其分为对照组(常规护理)和研究组(常规护理+多媒体教育联合临床路径情绪引导),各115例。比较两组的心理应激评分、生理应激指标、检查配合度及检查结局。结果检查后,两组的状态焦虑量表(S-AI)、恐惧视觉模拟评分法(FAVS)评分均显著降低,且研究组低于对照组(P<0.05)。检查后,两组的血管紧张素-Ⅱ(Ang-Ⅱ)、皮质醇(Cor)、去甲肾上腺素(NE)、β-内啡肽(β-EP)水平及心率(HR)、平均动脉压(MAP)均升高,但研究组显著低于对照组(P<0.05)。研究组的检查配合度与检查一次成功率高于对照组,检查时间短于对照组,进镜受阻率与不良反应发生率低于对照组(P<0.05)。结论多媒体教育联合临床路径情绪引导可有效缓解患者生理与心理应激,提高检查配合度,改善检查结局,在消化内镜检查护理中具有较高的应用价值。 Objective To explore the effects of multimedia education combined with clinical pathway emotional guidance on stress response,cooperation degree and examination outcome of patients undergoing digestive endoscopy.Methods A total of 230 patients who underwent digestive endoscopy in our hospital from February 2020 to February 2022 were selected as the research objects,and the patients were divided into control group(routine nursing)and study group(routine nursing+multimedia education combined with clinical pathway emotional guidance)according to the 1∶1 model,with 115 cases in each group.The psychological stress scores,physiological stress indexes,examination cooperation degree and examination outcome were compared between the two groups.Results After examination,the scores of State Anxiety Inventory(S-AI)and Fear Visual Analogue Scale(FAVS)in the two groups significantly decreased,and those in the study group were lower than the control group(P<0.05).After examination,the angiotension-Ⅱ(Ang-Ⅱ),cortisol(Cor),norepinephrine(NE),β-endorphin(β-EP)levels and heart rate(HR),mean arterial pressure(MAP)in the two groups increased,but those in the study group were significantly lower than the control group(P<0.05).The examination cooperation degree and one-time success rate of the study group were higher than those of the control group,the examination time was shorter than that of the control group,and the mirror obstruction rate and incidence of adverse reactions were lower than those of the control group(P<0.05).Conclusion Multimedia education combined with clinical pathway emotional guidance can effectively alleviate the physiological and psychological stress of patients,increase the coordination degree of examination,and improve examination outcome.It has high application value in the nursing of digestive endoscopy.
