叙事护理干预对食管癌化疗患者痛苦感的影响

Effect of narrative nursing intervention on the pain of patients with esophageal cancer undergoing chemotherapy

目的探讨叙事护理干预对食管癌化疗患者痛苦感的影响。方法选取我院2019年1月至2021年1月收治的80例食管癌化疗患者作为研究对象,按照电脑盲选法将其分为对照组和研究组,各40例。对照组给予常规护理干预,研究组在常规护理的基础上给予叙事护理干预。比较两组的干预效果。结果干预2周后,两组的P物质(SP)、前列腺素E_(2)(PGE_(2))及环氧化酶2(COX-2)水平低于干预前,且研究组低于对照组(P<0.05)。干预2周后,研究组的躯体疲乏、认知疲乏、情感疲乏评分及总分均低于干预前,且研究组低于对照组(P<0.05)。干预2周后,两组的白细胞分化抗原4阳性(CD4^(+))、白细胞分化抗原8阳性(CD8^(+))及CD4^(+)/CD8^(+)优于对照组(P<0.05)。研究组的疼痛度显著低于对照组(P<0.05)。结论叙事护理干预在食管癌化疗患者中的应用效果显著,可在减轻患者痛苦感方面发挥巨大作用,有利于缓解疲乏状态,增强免疫功能,值得推广和应用。

Objective To explore the effect of narrative nursing intervention on the pain of patients with esophageal cancer undergoing chemotherapy.Methods A total of 80 patients with esophageal cancer undergoing chemotherapy admitted in our hospital from January 2019 to January 2021 were selected as the research objects,and the patients were divided into control group and study group according to computer blind selection method,with 40 cases in each group.The control group was given routine nursing intervention,and the study group was given narrative nursing intervention on the basis of routine nursing.The intervention effects of the two groups were compared.Results After 2 weeks of intervention,the levels of substance P(SP),prostaglandin E_(2)(PGE_(2))and cyclooxygenase-2(COX-2)in the two groups were lower than those before intervention,and those in the study group were lower than the control group(P<0.05).After 2 weeks of intervention,the scores of physical fatigue,cognitive fatigue,emotional fatigue and total score in the two groups were lower than those before intervention,and those in the study group were lower than the control group(P<0.05).After 2 weeks of intervention,the cluster of differentiation 4 positive(CD4^(+)),cluster of differentiation 8 positive(CD8^(+))and CD4^(+)/CD8^(+)in the study group were better than those in the control group(P<0.05).The pain degree of the study group was significantly lower than that of the control group(P<0.05).Conclusion Narrative nursing intervention applied in patients with esophageal cancer undergoing chemotherapy has a significant effect,it plays a huge role in reducing the pain of patients,and is beneficial to alleviate fatigue state and enhance the immune function,which is worthy of promotion and application.
