六字诀呼吸操联合危重症专职护理对老年慢性阻塞性肺疾病急性加重期患者中医证候积分、肺功能及生活质量的影响

Effects of six-character breathing exercises combined with full-time nursing for critical illness on TCM syndrome score,lung function and quality of life in elderly patients with acute exacerbation of chronic obstructive pulmonary disease

目的探究六字诀呼吸操联合危重症专职护理在老年慢性阻塞性肺疾病急性加重期(AECOPD)患者中的应用效果及对中医证候积分、肺功能及生活质量的影响。方法选取2021年5月至2022年12月本院呼吸与危重症医学科二科收治的68例老年AECOPD患者为研究对象,根据护理干预方案将其分为对照组和观察组,每组34例。对照组实施危重症专职护理,观察组实施六字诀呼吸操联合危重症专职护理。比较两组护理前、后的中医证候积分、肺功能及生活质量。结果护理后,观察组的各项中医证候积分及总积分均显著低于对照组,差异具有统计学意义(P<0.05)。护理后,观察组的第1秒用力呼气容积(FEV_(1))占预计值的百分比(FEV_(1)%)、用力肺活量(FVC)、FEV_(1)/FVC均显著大于对照组,差异具有统计学意义(P<0.05)。护理后,观察组的圣·乔治呼吸问卷(SGRQ)各维度评分均显著低于对照组,差异具有统计学意义(P<0.05)。结论六字诀呼吸操联合危重症专职护理在老年AECOPD患者中的应用效果显著,可有效改善中医证候积分,增强肺功能,提高生活质量,值得大力推广。

Objective To explore the application effect of six-character breathing exercises combined with full-time nursing for critical illness in the elderly patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and its influences on TCM syndrome score,lung function and quality of life.Methods Sixty-eight elderly patients with AECOPD admitted in the Second Department of Respiratory and Critical Care Medicine of our hospital from May 2021 to December 2022 were selected as the study objects and divided into control group and observation group according to the nursing intervention plan,with 34 cases in each group.The control group was given full-time nursing for critical illness,and the observation group was given six-character breathing exercises combined with full-time nursing for critical illness.The TCM syndrome score,lung function and quality of life were compared between the two groups before and after nursing.Results After nursing,the TCM syndrome scores and total score of the observation group were significantly lower than those of the control group,and the differences were statistically significant(P<0.05).After nursing,the forced expiratory volume in one second(FEV_(1))as a percentage of the expected value(FEV_(1)%),forced vital capacity(FVC)and FEV_(1)/FVC in the observation group were significantly greater than those in the control group,and the differences were statistically significant(P<0.05).After nursing,the scores of each dimension of St.George's Respiratory Questionnaire(SGRQ)in the observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Six-character breathing exercises combined with full-time nursing for critical illness applied in elderly patients with AECOPD has remarkable effect,it can effectively improve the TCM syndrome score,enhance lung function and improve the quality of life,which is worthy of vigorous promotion.
