康复新液联合标准化疗方案治疗肺结核的效果及对患者血清α1酸性糖蛋白和触珠蛋白水平的影响

Effect of Kangfuxin liquid combined with standard chemotherapy scheme in the treatment of pulmonary tuberculosis and its effect on serum α1-acid glycoprotein and haptoglobin levels

目的探讨康复新液联合标准化疗方案(2HRZE/4HR方案)治疗肺结核的效果及对患者血清α1酸性糖蛋白和触珠蛋白水平的影响。方法选取河北省胸科医院2019年1月至2021年4月收治的188例肺结核患者。根据随机数字表法分为对照组和观察组,各94例。对照组给予2HRZE/4HR标准化疗方案及西药抗感染、止血、止咳化痰治疗,观察组在对照组基础上联合康复新液口服治疗,2组均连续治疗6个月。比较2组患者痰菌阴转率和CT病灶吸收、空洞变化、咯血症状缓解、不良反应发生情况,比较2组肺功能和炎症因子、T淋巴细胞亚群、血清α1酸性糖蛋白和触珠蛋白水平的变化。结果治疗2、5、6个月后观察组患者痰菌阴转率均高于对照组[83.0%(78/94)比69.2%(65/94)、93.6%(88/94)比83.0%(78/94)、97.9%(92/94)比88.3%(83/94)],差异均有统计学意义(均P<0.05)。治疗2及6个月后观察组的病灶吸收总有效率、空洞改善总有效率及治疗2个月后咯血症状缓解总有效率均高于对照组,治疗期间观察组不良反应总发生率低于对照组,治疗后观察组肺功能指标及炎症因子、T淋巴细胞亚群改善情况优于对照组,差异均有统计学意义(均P<0.05)。治疗后2组血清α1酸性糖蛋白和触珠蛋白水平均高于治疗前、且观察组高于对照组[(786±105)mg/L比(713±105)mg/L、(0.91±0.22)g/L比(0.78±0.21)g/L],差异均有统计学意义(均P<0.05)。结论康复新液联合标准化疗方案(2HRZE/4HR方案)治疗肺结核效果较好,不良反应发生率较低,同时可提高血清α1酸性糖蛋白和触珠蛋白水平。

Objective To explore the effect of Kangfuxin liquid combined with standard chemotherapy scheme(2HRZE/4HR scheme)in the treatment of pulmonary tuberculosis and its effect on serumα1-acid glycoprotein and haptoglobin levels.Methods From January 2019 to April 2021,188 patients with pulmonary tuberculosis admitted to Hebei Chest Hospital were selected.They were divided into control group and observation group according to random number table method,with 94 cases in each group.The control group was treated with 2HRZE/4HR standard chemotherapy scheme and western medicine for anti-infection,hemostasis,relieving cough and reducing expectoration.The observation group was treated with Kangfuxin liquid orally on the basis of the control group.Both groups were treated continuously for 6 months.The sputum negative conversion rate,CT focus absorption,cavity change,hemoptysis symptom relief and adverse reactions were compared between the two groups.The lung function,inflammatory factors,T lymphocyte subsets,serumα1-acid glycoprotein and haptoglobin levels were compared between the two groups.Results After 2,5 and 6 months of treatment,the sputum negative conversion rates in the observation group were higher than those in the control group[83.0%(78/94)vs 69.2%(65/94),93.6%(88/94)vs 83.0%(78/94),97.9%(92/94)vs 88.3%(83/94)](all P<0.05).The total effective rates of focus absorption,cavity improvement at the end of 2 and 6 months of treatment and the total effective rate of hemoptysis symptom relief at the end of 2 months of treatment in the observation group were higher than those in the control group;the total incidence rate of adverse reactions in the observation group was lower than that in the control group during the treatment,the improvements of lung function indexes,inflammatory factors and T lymphocyte subsets in the observation group were better than those in the control group after treatment(all P<0.05).The levels ofα1-acid glycoprotein and haptoglobin in both groups after treatment were higher than those before treatment,and the levels in the observation group were higher than those in the control group[(786±105)mg/L vs(713±105)mg/L,(0.91±0.22)g/L vs(0.78±0.21)g/L](all P<0.05).Conclusion Kangfuxin liquid combined with standard chemotherapy scheme(2HRZE/4HR scheme)has a better effect in the treatment of pulmonary tuberculosis,with low incidence rate of adverse reaction,and can also improve serumα1-acid glycoprotein and haptoglobin levels.
