摘要
目的:探讨阴道微环境与高危型人乳头瘤病毒(HR-HPV)持续性感染相关性研究。方法:选取2021年7月~2022年3月郴州市第一人民医院中心医院妇科门诊就诊自愿行阴道微环境检测和HPV检测的873例女性作为研究对象,对于阴道微环境检测结果异常的患者分析异常阴道微环境改变情况与HR-HPV持续感染的关系,分析功能指标检测结果阳性和阴性者HR-HPV感染率情况。结果:在873例检测者中,289例(33.10%)阴道微环境正常,584例(66.90%)阴道微环境异常,细菌性阴道病(BV)、假丝酵母菌性阴道炎(VVC)和需氧菌性阴道炎(AV)阳性者HR-HPV感染率高于阴性者,差异有统计学意义(P<0.05),单纯过氧化氢(H_(2)0_(2))缺乏者、单纯H_(2)0_(2)缺乏+白细胞酯酶(LE)阳性和阴道微环境正常者HR-HPV感染率分别为24.36%、29.52%和7.96%。唾液酸苷酶(Snase)、白细胞酯酶(LE)、凝固酶(GA)和葡萄糖醛酸苷酶(Gus)阳性者HR-HPV感染率明显高于Snase、LE、GA和Gus阴性者HR-HPV感染率,差异有统计学意义(P<0.05),pH值>4.5和白细胞计数(WBC)>10时HR-HPV感染率高于pH值<4.5和WBC<10时HR-HPV感染率,差异有统计学意义(P<0.05)。结论:BV、VVC、AV、pH值改变等阴道微环境异常变化与HR-HPV持续感染有关。
Objective To investigate the correlation between vaginal microenvironment and HR-HPV persistent infection.Method From July 2021 to March 2022,873 women who visited the gynecological outpatient department of our hospital and underwent voluntary vaginal microenvironment testing and HPV testing were selected as the research subjects,and the abnormal vaginal microenvironment changes were analyzed for patients with abnormal vaginal microenvironment testing results.The relationship between the situation and HR-HPV persistent infection was analyzed,and the HR-HPV infection rate of positive and negative functional index test results was analyzed.Results Among the 873 testers,289 cases(33.10%)had normal vaginal microenvironment,and 584 cases(66.90%)had abnormal vaginal microenvironment.The HR-HPV infection rate of BV,VVC and AV positive patients was higher than that of negative ones(P<0.05),the HR-HPV infection rates were 24.36%,29.52%and 7.96%in patients with pure H_(2)0_(2)deficiency,pure H_(2)0_(2)deficiency+LE positive and normal vaginal microenvironment,respectively.The HR-HPV infection rate in Snase,LE,GA and Gus positive patients was significantly higher than that in Snase,LE,GA and Gus negative patients(P<0.05).The infection rate was higher than HR-HPV infection rate when pH value was lower than 4.5 and WBC was lower than 10(P<0.05).Conclusion Abnormal changes in vaginal microenvironment such as BV,VVC,AV,and PH value changes are related to persistent HR-HPV infection.
