基于MRI影像组学特征对乳腺癌淋巴血管浸润的预测价值

Predictive value of MRI imaging histological features in lymphangiogenesis invasion of breast cancer

目的探讨MRI影像组学特征对预测乳腺癌有关淋巴血管浸润(LVI)的价值。方法选取病理确诊为乳腺浸润导管癌的患者80例,按照是否发生LVI分成发生组22例和未发生组58例,比较两组的临床资料和MRI表现差异。结果两组在年龄、文化程度、肿瘤背景实质强度程度、肿瘤最大直径方面差异无统计学意义(P>0.05);但在有无腋窝中淋巴结肿大、有无瘤周水肿、ADC值方面差异有统计学意义(P<0.05)。将有差异资料带入Logistic回归方程计算,得出腋窝中淋巴结肿大、瘤周水肿、ADC值均是影响乳腺癌LVI的主要因素,OR值>1。结论腋窝中淋巴结肿大、瘤周水肿、ADC值是乳腺癌LVI的影响因素,MRI有关影像组学特征对乳腺癌LVI均有较高的预测效能。

Objective To analyze the value of magnetic resonance imaging(MRI)in predicting lymphovascular invasion(LVI)in breast cancer.Methods Retrospective analysis was carried out on the clinical data of 80 patients with breast infiltrating ductal carcinoma who had undergone preoperative MRI examination in our hospital from January 2021 to October 2022 and were pathologically diagnosed as breast infiltrating ductal carcinoma.According to whether LVI occurred,22 patients were divided into the occurrence group and 58 patients were divided into the non occurrence group.The differences in clinical data and MRI performance between the two groups were compared.The data with differences were brought into the Logistic regression equation for calculation,and the main factors affecting LVI were obtained,the predictive value of MRI in LVI was analyzed.Results There was no significant difference between the two groups in terms of age,education level,tumor background,solid strength,and maximum tumor diameter(P>0.05);However,there were significant differences in the presence or absence of axillary lymph node enlargement,peritumoral edema and ADC value(P<0.05).The difference data were brought into the Logistic regression equation to calculate,and it was concluded that lymph node enlargement,peritumoral edema,ADC value were the main factors affecting LVI in breast cancer,with OR value>1 respectively.Conclusion Axillary lymph node enlargement,peritumoral edema,and ADC value are the influencing factors of LVI in breast cancer.These MRI related imageomics characteristics have a high predictive effect on LVI in breast cancer,and are expected to become important indicators to predict LVI status before surgery.
