声门上型喉癌颈清扫范围研究进展

喉癌是头颈部常见的恶性肿瘤,其中声门上型约占40%。声门上型喉癌极易发生颈部淋巴结的转移。根据触诊、超声等术前检查未发现淋巴结转移的称为cN0。声门上型喉癌常转移至病变同侧淋巴结,以Ⅱ区受累最为常见,Ⅲ区其次,Ⅳ区受累较少见,Ⅴ区淋巴结很少受累。且转移率随着肿瘤T分期的升高而逐渐升高。对声门上型喉癌行颈清扫术至关重要,对于声门上型喉癌的颈清扫范围,国内外目前尚无统一的定论,本文就声门上型喉癌如何选择合适的颈清扫范围作一综述。

Laryngeal cancer is a common malignant tumor of head and neck,of which supraglottic type accounts for about 40%.Supraglottic laryngeal carcinoma is prone to cervical lymph node metastasis.According to palpation,ultrasound and other preoperative examinations,those who do not find lymph node metastasis are called cN0.Supraglottic laryngeal carcinoma often metastasizes to the ipsilateral lymph nodes.The involvement of zone II is the most common,followed by zone III,rarely involved in Zone IV,and rarely involved in Zone V lymph nodes.And the metastasis rate increased gradually with the increase of tumor T stage.Neck dissection for supraglottic laryngeal cancer is very important.There is no unified conclusion about the scope of neck dissection for supraglottic laryngeal cancer at home and abroad.This paper reviews how to choose the appropriate scope of neck dissection for supraglottic laryngeal cancer.
