磁共振扩散峰度成像在肺部肿瘤良恶性诊断中的效能 被引量:1

Efficacy of imaging-related parameters of magnetic resonance diffusion kurtosis imaging(DKI)in diagnosis of benign and malignant lung tumors

摘要 目的:探讨磁共振扩散峰度成像(DKI)在肺部肿瘤良恶性诊断中的效能。方法:选取2018年4月至2022年10月该院收治的70例疑似肺部肿瘤患者作为研究对象,患者均接受磁共振DKI检查,获取磁共振DKI相关参数[平均峰度系数(MK)、平均扩散系数(MD)]水平,以病理检查结果为“金标准”,采用受试者工作特征(ROC)曲线分析磁共振DKI检查在肺部肿瘤良恶性诊断中的效能。结果:常规多层螺旋计算机断层扫描共检出肺部肿瘤灶77个。病理检查结果显示,77个肺部肿瘤灶中良性病灶33个,占42.86%,其中肺炎性假瘤24个,肺错构瘤7个,肺纤维瘤2个;恶性病灶44个,占57.14%,其中肺腺癌31个,肺鳞癌13个。恶性病灶MK水平高于良性病灶,MD水平低于良性病灶,差异有统计学意义(P<0.05);ROC曲线分析显示,MK、MD水平单项及联合诊断肺部肿瘤良恶性的曲线下面积分别为0.830、0.815、0.891,均有一定的诊断价值,其中联合检查诊断的效能最高。结论:磁共振DKI检查的影像学相关参数在肺部肿瘤良恶性诊断中具有较高的效能。 Objective:To investigate efficacy of imaging-related parameters of magnetic resonance diffusion kurtosis imaging(DKI)in diagnosis of benign and malignant lung tumors.Methods:70 patients with suspected lung tumors admitted to the hospital from April 2018 to October 2022 were selected as the study subjects.All patients underwent magnetic resonance DKI examination to obtain magnetic resonance DKI-related parameters[mean kurtosis coefficient(MK),mean diffusion coefficient(MD)]levels.Using the results of pathological examination as the“gold standard”,the efficacy of magnetic resonance DKI in the diagnosis of benign and malignant lung tumors was analyzed by receiver operating characteristic(ROC)curve.Results:A total of 77 lung tumors were detected by conventional multi-slice spiral computed tomography.The pathological examination showed that there were 33 benign lesions in the 77 lung tumors,accounting for 42.86%,including 24 pulmonary inflammatory pseudotumors,7 pulmonary hamartomas and 2 pulmonary fibromas;there were 44 malignant lesions,accounting for 57.14%,including 31 lung adenocarcinomas and 13 lung squamous cell carcinomas.The MK level of the malignant lesions was higher than that of the benign lesions,and the MD level was lower than that of the benign lesions,and the differences were statistically significant(P<0.05).The ROC curve showed that the areas under the curve of the MK and MD levels in the diagnosis of benign and malignant lung tumors were 0.830,0.815 and 0.891,separately,which had certain diagnostic value,and the diagnostic efficacy of combined examination was the highest.Conclusions:The imaging parameters of magnetic resonance DKI examination have high efficacy in the diagnosis of benign and malignant lung tumors.
