中性粒细胞明胶酶相关脂质运载蛋白在猫急性肾损伤中的诊断价值

Diagnostic Value of Neutrophil Gelatinase-associated Lipocalin in Cats with Acute Kidney Injury

为初步研究中性粒细胞明胶酶相关脂质运载蛋白(NGAL)在猫急性肾损伤(AKI)中的诊断价值,本试验采集至中国农业大学动物医院就诊猫的血液和尿液样本,根据临床检查结果将其分为健康组、尿道梗阻(UO)组、慢性肾病(CKD)组、慢性肾病急性发作(ACKD)组和AKI组,检测血清中性粒细胞明胶酶相关脂质运载蛋白(sNGAL)、尿液中性粒细胞明胶酶相关脂质运载蛋白(uNGAL)浓度和尿液中性粒细胞明胶酶相关脂质运载蛋白与尿液肌酐比(UNCR),并分析三者与血清肌酐(sCr)的相关性。结果显示,sNGAL在组间无显著差异(P>0.05);健康组uNGAL和UNCR与CKD组、ACKD组、AKI组均存在极显著差异(P<0.01),但CKD组、ACKD组和AKI组各组间无显著差异(P>0.05);uNGAL、UNCR与sCr均呈弱相关(r_(uNGAL)=0.363,r_(UNCR)=0.385)。结果表明,sNGAL不能作为诊断猫AKI的生物标志物,uNGAL和UNCR对猫AKI的诊断具有重要的临床意义,但不能区分AKI和CKD。

This study aimed to investigate the diagnostic value of neutrophil gelatinase-associated lipocalin(NGAL)in cats with acute kidney injury(AKI).Blood and urine samples were collected from cats presented at the Veterinary Teaching Hospital of China Agricultural University.Based on clinical examination results,the cats were divided into healthy group,urethral obstruction(UO)group,chronic kidney disease(CKD)group,acute-on-chronic kidney disease(ACKD)group,and AKI group.The concentrations of serum NGAL(sNGAL),urine NGAL(uNGAL),and the ratio of urine NGAL to urine creatinine(UNCR)were measured,and their correlation with serum creatinine(sCr)were analyzed.The results showed no significant difference in sNGAL among the groups(P>0.05).Significant differences were observed in uNGAL and UNCR between the healthy group and the CKD,ACKD,and AKI groups(P<0.01),but no significant difference were found among the CKD,ACKD,and AKI groups(P>0.05).uNGAL,UNCR,and sCr showed weak correlations(r_(uNGAL)=0.363,r_(UNCR)=0.385).These findings suggest that sNGAL is not a reliable biomarker for diagnosing AKI in cats,uNGAL and UNCR have important clinical significance in the diagnosis of AKI in cats,but they cannot differentiate AKI from CKD.
