RVO患者房水MCP-1、IL-8表达与黄斑水肿相关分析

摘要 目的研究视网膜静脉阻塞(Retinal vein occlusion,RVO)患者房水中白介素-8(Interleukin-8、IL-8)、单核细胞趋化因子-1(MCP-1)的水平与RVO继发黄斑水肿之间的关系。方法选取2021年7月至2023年2月在我院眼科住院行抗血管内皮生长因子(Vascular endothelial growth factor,VEGF)治疗的视网膜静脉阻塞型黄斑水肿(Macular edema due to retinal vein occlusion RVO-ME)为研究组(n=55),对照组为同时段于我院行白内障手术的患者(n=15)。房水采集后,用ELISA检测房水中IL-8、MCP-1的浓度,收集被研究者的中央视网膜厚度(central retinal thickness,CRT)、最佳矫正视力(best corrected visual acuity,BCVA)、视网膜下积液高度(subfoveal serous retinal thickness,SRT)。按照不同的分组标准进行数据分析。结果实验组的高血压患病率、IL-8、MCP-1的水平均显著高于对照组(P<0.05)。房水中MCP-1的含量视网膜中央静脉阻塞性黄斑水肿(Macular edema due to central retinal vein occlusion,CRVO-ME)组高于视网膜分支静脉阻塞黄斑水肿(Macular edema due to branch retinal vein occlusion,BRVO-ME)组(P=0.017);BRVO-ME组内的相关性分析:BCVA(logMAR)与CRT_(基线)之间呈现正相关(P<0.001,r=0.542),即BCVA与CRT_(基线)之间呈现负相关;MCP-1水平与CRT_(基线)呈正相关(P<0.001,r=0.598);IL-8水平与CRT_(基线)呈正相关(P=0.035,r=0.311);CRT_(基线-1) W/CRT_(基线)与CRT_(基线)及MCP-1的含量呈正相关(P=0.009,r=0.396),与IL-8的含量的相关性无统计学意义(P>0.05);IL-8、MCP-1的房水表达以及CRT_(基线-1) W/CRT_(基线)在SRD、CME、DRT三组之间差异无统计学意义(P>0.05)。SRD型BRVO-CME中SRT与IL-8水平呈正相关(P=0.028,r=0.531)。结论炎症因子IL-8和MCP-1参与RVO-ME的发生发展,MCP-1、IL-8含量越高,CRT_(基线)的值越大,ME程度就越严重,患者的BCVA就越差;CRT_(基线)越大,抗-VEGF治疗后黄斑水肿恢复越快。在SRD型中,房水中IL-8的含量越大,SRT就值就越高。 Objective To investigate the relationship between the levels of IL-8 and MCP-1 in aqueous humor and macular edema secondary to Retinal vein occlusion(RVO)in patients with RVO.Methods RVO-ME patients who received anti-VEGF treatment in our hospital from July 16,2021 to February 6,2023 were selected as the experimental group(n=55),while the control group included patients who underwent cataract surgery in our hospital at the same time(n=15).After aqueous humor collection,the concentrations of IL-8 and MCP-1 in aqueous humor were detected using ELISA,and the patients'CRT,BCVA,and SRT were collected.Conduct data analysis according to different grouping.Results The incidence of hypertension,levels of IL-8 and MCP-1 in the experimental group were significantly higher than those in the control group(P<0.05).The content of MCP-1 in aqueous humor of the CRVO-ME group was higher than that of the BRVO-ME group(P=0.017);Correlation analysis within the BRVO-ME group:BCVA(logMAR)showed a positive correlation with CRT baseline(P<0.001,R=0.542),indicating a negative correlation between BCVA and CRT baseline;The level of MCP-1 is positively correlated with the baseline of CRT(P<0.001,R=0.598);The level of IL-8 is positively correlated with the baseline of CRT(P=0.035,R=0.311);CRT baseline-1W/CRT baseline showed a positive correlation with CRT baseline and MCP-1 content(P=0.009,R=0.396),but there was no statistically significant correlation with IL-8 content(P>0.05);There was no statistically significant difference in the expression of IL-8 and MCP-1 in aqueous humor,as well as the CRT baseline-1W/CRT baseline among the SRD,CME,and DRT groups(P>0.05).There is a positive correlation between SRT and IL-8 levels in SRD type BRVO-CME(P=0.028,R=0.531).Conclusion Inflammatory factors IL-8 and MCP-1 are involved in the occurrence and development of RVO-ME.The higher the content of MCP-1 and IL-8 showed a correlation with the greater the baseline value of CRT,the more severe the degree of ME,and the worse the BCVA of the patient;The larger the CRT baseline showed a correlation with the faster the recovery of macular edema after anti-VEGF treatment.In the SRD type,the higher content of IL-8 in aqueous humor showed a correlation with the higher SRT value.
