新标准化管理体系在膀胱灌注病人中的应用

New standardized management system in bladder perfusion patients

目的探讨新标准化管理体系在胱肿瘤电切术后胱灌注治疗病人的应用效果。方法选取2021年1月至2021年12月间在安徽省某三甲医院泌尿外科行膀胱肿瘤电切术且术后行膀胱灌注治疗的62例病人作为对照组,实施常规管理体系;选取2022年1月至2022年12月间在该院进行相同诊疗的63例病人作为观察组,实施新标准化管理体系,按照建立病房+微创治疗中心联动管理小组、建立标准化作业流程、建立特色健康教育与随访流程等步骤为病人实施护理干预。比较两组病人不良反应发生率、治疗依从性、知识掌握情况、焦虑水平及灌注治疗满意度。结果观察组病人治疗依从性、每次灌注后药物保留时间达标情况、灌药知识掌握情况、灌注治疗满意度均高于对照组(P<0.05);观察组病人的灌注后不良反应发生率及焦虑水平均低于对照组(P<0.05)。结论新标准化膀胱灌注管理体系是一项基于科学研究较为系统全面的、切实可行的管理模式,可有效提高病人灌注依从性及灌药知识掌握程度,降低灌注不良反应的发生及病人焦虑水平,提升病人满意度。

Objective To explore the effect of the new standardized management system in patients treated with cyst perfusion after cystectomy for bladder tumors.Methods Sixty-two patients who underwent bladder tumor electrosurgery and postoperative bladder irrigation treatment in the Department of Urology of a tertiary hospital in Anhui Province from January 2021 to December 2021 were selected as the control group,and the conventional management system was implemented.Sixty-three patients who underwent the same treatment in this hospital from January 2022 to December 2022 were selected as the observation group,and the new standardized management system was implemented,according to the steps of establishing a Ward+Minimally invasive treatment center linkage management group,establishment of standardized operation process,establishment of characteristic health education and follow-up process,and other steps to implement nursing interventions for patients.The complication rate,treatment compliance,knowledge acquisition,anxiety level and nursing satisfaction in the two groups were compared.Results Patients in the observation group had higher compliance with treatment,attainment of drug retention time after each perfusion,knowledge of drug perfusion,and satisfaction with care than the control group(P<0.05).the incidence of complications and anxiety levels after perfusion were lower in the observation group than in the control group(P<0.05).Conclusion The new standardized bladder perfusion management system for bladder cancer patients throughout the period from hospitalization,perfusion in the minimally invasive urology center to follow-up is a more systematic,and comprehensive and practical management model based on scientific research,which can effectively improve patients'compliance with perfusion and knowledge of perfusion,reduce the occurrence of perfusion complications and patient anxiety level,and improve patient satisfaction.
