电针联合中药熏洗在类风湿关节炎患者中的应用及效果观察

The application and Effect Observation of the combination of electroacupuncture and traditional Chinese medicine fumigation and washing in patients with rheumatoid arthritis

目的探讨电针联合中药熏洗在类风湿关节炎(Rheumatoid arthritis,RA)患者中的应用及效果。方法便利抽样法选取2018年1月至2020年5月收治于合肥市某三甲医院风湿科RA患者140例,随机分为实验组及对照组。对照组患者接受常规治疗与护理,实验组患者在对照组基础上实施电针联合中药熏洗。两组患者均干预3个月,观察并比较两组患者的关节功能优良率、关节压痛指数、关节肿胀指数、晨僵时间、类风湿因子(Rheumatoid factors,RF)、血沉(Erythrocyte sedimentation rate,ESR)、C反应蛋白(C-reactive protein,CRP)、VAS评分、DAS28评分、睡眠质量。结果干预3个月后实验组患者关节功能优良率明显优于对照组患者(P<0.05)。干预中及干预后两组患者的关节压痛指数、关节肿胀指数、晨僵时间、RF、ESR、CRP、VAS评分、DAS28评分均较干预前明显降低,且实验组患者均显著低于对照组患者(P<0.05)。干预3个月后两组患者的PSQI评分均低于干预前、干预1个月、干预2个月、干预3个月,且实验组患者显著低于对照组(P<0.05)。结论电针联合中药熏洗可有效改善RA患者的关节功能,提高患者的睡眠质量,且操作简单,患者接受度高,可在病房推广实施。

Objective To explore the application and effect of the combination of electroacupuncture and traditional Chinese medicine fumigation and washing in patients with rheumatoid arthritis(RA).Methods A Convenient sampling method was used to select 140 RA patients admitted to the Rheumatology Department of a tertiary hospital in Hefei from January 2018 to May 2020.They were randomly divided into an experimental group and a control group.The control group received routine treatment and care.Compared with the control group,the experimental group added electroacupuncture combined with traditional Chinese medicine fumigation and washing treatment.The excellent and good rate of joint function,joint tenderness index,joint swelling index,morning stiffness time,RF,ESR,CRP,VAS score,DAS28 score,and sleep quality were observed and compared after intervention for 3 months.Results Compared with the control group,the excellent and good rate of joint function in the experimental group was significantly increased after intervention for 3 months(P<0.05).Compared with before the intervention,the joint tenderness index,joint swelling index,morning stiffness time,RF,ESR,CRP,VAS score,and DAS28 score of the two groups of patients were significantly reduced during and after the intervention,and the experimental group was significantly lower than the control group(P<0.05).After 3 months of intervention,the PSQI scores of both groups were lower than before intervention,and 1 month,2 months,and 3 months after intervention,the experimental group were significantly lower than the control group(P<0.05).Conclusion The joint function and sleep quality of RA patients were improved after the combination of electroacupuncture and traditional Chinese medicine fumigation and washing.The combination of electroacupuncture and traditional Chinese medicine fumigation and washing has the characteristics of simple operation and high patient acceptance,which can be promoted and implemented in wards.
