摘要
目的探讨超声造影技术联合超声乳腺影像学报告及数据系统(BI-RADS)分类对诊断乳腺良恶性肿瘤的临床价值。方法从2022年1—10月在福建省诏安县总医院接受乳腺检查的517例受检者中选取162例经病理学检查且确诊具有乳腺肿瘤病灶的患者作为研究对象,分别对患者进行临床病理学、超声造影及BI-RADS分类法诊断。观察患者病理学诊断结果,比较超声造影、BI-RADS分类法及超声造影联合BI-RADS分类法对乳腺肿瘤良恶性质的诊断效能。结果超声造影联合BI-RADS分类法对乳腺肿瘤良恶性鉴别的灵敏度为90.00%,特异度为94.26%,准确性为93.21%,阳性预测值为83.72%,阴性预测值为96.64%,其特异度、准确性、阳性预测值及阴性预测值均明显高于BI-RADS分类法,差异均有统计学意义(P<0.05)。超声造影联合BI-RADS分类法对乳腺肿瘤良恶性鉴别的灵敏度、特异度、准确性、阳性预测值及阴性预测值略高于超声造影,但差异均无统计学意义(P>0.05)。结论超声造影联合BI-RADS分类对提高临床鉴别乳腺良恶性肿瘤的效能具有积极意义,值得临床推广应用。
Objective To explore the clinical value of contrast-enhanced ultrasound technology combined with ultrasound breast imaging report and data system(BI-RADS)classification in the diagnosis of breast benign and malignant tumors.Methods From January to October 2022,162 patients with breast tumor lesions confirmed by pathological examination were selected as the study subjects from 517 patients who underwent breast examination in Fujian Province Zhao'an County General Hospital,and the patients were respectively diagnosed by clinical pathology,contrast-enhanced ultrasound and BI-RADS classification.The pathological diagnosis results of patients were observed,and the diagnostic efficacy of contrast-enhanced ultrasound,BI-RADS classification and contrast-enhanced ultrasound combined with BI-RADS classification in the benign and malignant nature of breast tumors were compared.Results The sensitivity of contrast-enhanced ultrasound combined with BI-RADS classification for the identification of benign and malignant breast tumors was 90.00%,the specificity was 94.26%,the accuracy was 93.21%,the positive predictive value was 83.72%,and the negative predictive value was 96.64%,and its specificity,accuracy,positive predictive value and negative predictive value were significantly higher than those of BI-RADS classification,and the differences were statistically significant(P<0.05).The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of contrast-enhanced ultrasound combined with BI-RADS classification for the indentification of benign and maliganant breast were slighty higher than those of contrast-enhanced ultrasound,but the differences were not statistically significant(P>0.05).Conclusion The combination of contrast-enhanced ultrasound and BI-RADS classification had positive significance in improving the efficiency of clinical differentiation of breast benign and malignant tumors,and was worthy of clinical popularization and application.
