预见性护理模式在肺癌免疫治疗中的应用

Application of predictive nursing mode in immunotherapy of lung cancer

目的研究观察预见性护理模式在驱动基因阴性非小细胞肺癌患者使用免疫检查点(PD-1/PD-L1)抑制剂所发生的免疫不良反应中的应用。方法选取2019年6月—2021年6月天津市环湖医院收治的驱动基因阴性非小细胞肺癌接受免疫检查点抑制剂治疗的患者100例,依据随机双盲法分为研究组和对照组各50例,研究组患者给予预见性护理,对照组患者给予常规护理。比较两组免疫治疗后的不良反应发生率。结果研究组不良反应发生率为6.00%,其中皮疹1例,胃肠道反应2例,未出现肝脏及肺部毒性;对照组不良反应发生率为20.00%,其中皮疹4例,胃肠道反应4例,肝脏毒性1例,肺部毒性1例;研究组不良反应发生率明显低于对照组,差异有统计学意义(P<0.05)。结论对于非小细胞肺癌患者,使用预见性护理模式进行护理,可以有效减少患者在免疫治疗中的不良反应,从而提高护理质量,减轻患者痛苦。

Objective To study the application of predictable nursing mode in treating adverse reactions happened in using immune checkpoint(PD-1/PD-L1)inhibitors in driver gene-negative non-small cell lung cancer patients.Methods A total of 100 patients with driver gene-negative non-small cell lung cancer treated with immune checkpoint inhibitors from June 2019 to June 2021 in Tianjin Huanhu Hospital were selected and divided into the study group and the control group according to random double-blind method,with 50 cases in each group.Patients in the study group were given predictive nursing,and patients in the control group were given routine nursing.The incidences of adverse reactions after immunotherapy between the two groups were compared.Results The incidence of adverse reactions in the study group was 6.00%,including 1 case of rash,2 cases of gastrointestinal reactions,and no liver and pulmonary toxicity;the incidence of adverse reactions in the control group was 20.00%,including 4 cases of rashes,4 cases of gastrointestinal reactions,1 case of liver toxicity,and 1 case of pulmonary toxicity;the incidence of adverse reactions in the study group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion For patients with non-small cell lung cancer,using the predictive nursing mode for nursing could effectively reduce the adverse reactions of patients in immunotherapy,thereby improving the quality of care and reducing the pain of patients.
