经口腔前庭入路腔镜甲状腺手术在甲状腺微小乳头状癌的应用

Application of transoral endoscopic thyroidectomy via vestibular approach in thyroid micropapillary carcinoma

目的探讨经口腔前庭入路腔镜甲状腺手术(transoral endoscopic thyroidectomy via vestibular approach,TOETVA)治疗甲状腺微小乳头状癌的应用价值。方法本次纳入对象为郑州市中心医院甲状腺外科2019年9月—2020年9月收治的甲状腺微小乳头状癌患者67例,依据手术方式分组,将实行传统甲状腺开放术的32例作为对照组,将实行经口腔前庭入路腔镜甲状腺手术的35例作为观察组,比较两组手术效果。结果两组术中出血量、住院时间及中央区淋巴结清扫数量比较差异无统计学意义(P>0.05);观察组手术时间及术后美容满意度均优于对照组(P<0.05);观察组术后并发症发生率为8.6%,明显低于对照组(28.1%)(P<0.05)。结论TOETVA术治疗甲状腺微小乳头状癌有效且安全,且美容满意度高,符合患者需求,值得推广。

Objective To investigate the clinical value of transoral endoscopic thyroidectomy via vestibular approach(TOET-VA)in the treatment of thyroid micropapillary carcinoma.Methods The clinical data of 67 patients with thyroid micropapil-lary carcinoma who were admitted to Zhengzhou Central Hospital between September 2019 and September 2020 were retrospec-tively analyzed.According to different surgical methods,they were divided into traditional open thyroidectomy group(control group,n=32)and transoral endoscopic thyroidectomy group(observation group,n=35).The perioperative data and fllow-up status of the two groups were compared.Results There was no significant difference in intraoperative blood loss,length of hospital stay and number of central lymph nodes dissection between the two groups(P>0.05).The operation time and post-operative aesthetic satisfaction in observation group were better than those in control group(P<0.05).The incidence of post-operative complications in the observation group was 8.6%,which was significantly lower than that in the control group(28.1%)(P<0.05).Conclusion TOETVA is effective and safe in the treatment of thyroid micropapillary carcinoma,with high cosmetic satisfaction,which meets the needs of patients and is worthy of promotion.
