从“郁”论治甲状腺结节

Treatment of Thyroid Nodule from the Perspective of‘Depression’

近年来,甲状腺结节的发病率呈逐年升高趋势,西医学对于甲状腺结节的治疗主要以鉴别结节性质及定期随访观察为主,在疾病发展过程中并没有过多的干预,而中医对于甲状腺结节的治疗具有一定优势,可以达到“未病先防”“既病防变”的临床疗效。本文尝试从郁角度出发,深入探讨甲状腺结节的中医病机,提出行气解郁是治疗甲状腺结节的前提条件,化痰祛瘀、消癥散结是治疗甲状腺结节的重要原则。在解郁、化痰、活血等方法的基础上注意补益药物的运用,使得气血调和,五脏得养,阴阳平衡,为甲状腺结节治疗提供丰富的理论依据。

In recent years,the incidence of thyroid nodules has been increasing year by year.The treatment of thyroid nodules in Western medicine is mainly based on the identification of nodule properties and regular follow-up observation,and there are not many interventions during the development of this disease.However,traditional Chinese medicine(TCM)has some advantages in the treatment of this disorder,which can achieve clinical efficacy of‘prevention before disease onset’and‘controlling development of existing disease’.This paper attempts to deeply discuss the pathogenesis of thyroid nodules in TCM from the perspective of depression,and puts forward that promoting qi to relieve depression is a prerequisite for treating thyroid nodules,and that the important principles for treating thyroid nodules are to resolve phlegm,dispel stasis,and dissipate mass.On the basis of the methods such as relieving depression,resolving phlegm,and activating blood,attention should also be paid to the use of tonifying drugs,in order to harmonize qi and blood,nourish the five organs,and balance yin and yang,and thus,this paper provides rich theoretical basis for the treatment of thyroid nodules.
