“脾气亏损-线粒体能量代谢障碍-艾滋病相关疲乏”的病机关联假说构建

摘要 疲乏是艾滋病病毒(HIV)感染者的典型症状,严重影响了该类患者的正常工作与生活。线粒体作为机体的“能量工厂”,其内发生的能量代谢维持机体日常生命活动。常规的抗病毒治疗和HIV病毒本身均会造成线粒体损伤,能量代谢障碍易导致疲乏症状。脾脏是HIV感染后机体主要受损脏腑,脾气亏损伴随艾滋病发病的全过程,中医认为脾气散精,是五脏六腑能量之源,人的生长发育、生命活动都要以脾气充盈为前提条件。且根据现代研究,脾与线粒体能量代谢在来源、功能等方面契合度较高。故本文以艾滋病关键病机脾气亏损为契机,提出“脾气亏损-线粒体能量代谢障碍-艾滋病相关疲乏”的病机关联假说,同时推测以健脾益气法为指导思想研发的益艾康胶囊治疗艾滋病相关疲乏的潜在机制,为艾滋病相关疲乏的治疗提供新思路。 Fatigue is a typical symptom of HIV infected patients,which seriously affects their normal work and life.Mitochondria,as the‘factory of energy’within body,its energy metabolism maintain daily life activities.Conventional antiviral therapy and HIV virus itself can cause mitochondrial damage,and the disorder of its energy metabolism is easy to result in fatigue.Spleen is the main organ to be affected after HIV infection,and the deficiency of Spleen Qi is existing during the whole process of AIDS.TCM believes that Spleen Qi is responsible for dispersing essence,which is the source of energy of five-Zang organs and six-Fu organs.The sufficiency of Spleen Qi is the precondition of the growth,development,and life activities of human body.According to modern researches,Spleen and mitochondrial energy metabolism are highly consistent in source and function.Therefore,taking the key pathogenesis of Spleen Qi deficiency in AIDS as a discussion point,this study put forward the pathogenesis correlation hypothesis of‘Spleen Qi deficiency-mitochondrial energy metabolism disorder-AIDS related fatigue’,and discussed potential mechanism of Yiaikang Capsule in treating AIDS related fatigue,guided by the idea of invigorating Spleen and replenishing Qi,and provided a new way of treating AIDS related fatigue.
