三种锤拍排气法在血液灌流器预冲中的应用效果

摘要 目的探讨三种锤拍排气法在血液灌流器预冲中的临床效果。方法回顾性选取2021年7月至12月广州中医药大学第二附属医院透析科收治的75例行血液灌流的维持性血液透析患者,按照灌流器预冲的方法分为传统锤拍法组、一分钟排气法组和锤拍振动法组,每组25例。比较三组血液灌流器需要的排气时间及气体清除效果。结果三组残余气泡量比较,一分钟排气法组在预冲200 ml生理盐水时,灌流器残余气泡量少于传统锤拍法组与锤拍振动法组,差异有统计学意义(P<0.05);锤拍振动法组与传统锤拍法组残余气泡量比较,差异无统计学意义(P>0.05)。一分钟排气法组和锤拍振动法组在预冲500ml生理盐水时与预冲1000ml生理盐水时,灌流器残余气泡量少于传统锤拍法组,差异有统计学意义(P<0.05);一分钟排气法组与锤拍振动法组残余气泡量比较,差异无统计学意义(P>0.05)。预冲1500ml盐水时,三组残余气泡量比较,差异无统计学意义(P>0.05)。三组排气时间比较,锤拍振动法组用时最短,其次为一分钟排气法组,传统锤拍法组用时最长,三组间比较差异均有统计学意义(P<0.05)。三组未发生预冲异常。结论锤拍振动法预冲灌流器时间最短,排气效果满意,而一分钟排气法排尽大气泡效果最佳,排气效果较满意。 Objective To explore the clinical effect of three kinds of hammer exhaust method in the pre-flush of hemoperfusion.Methods Seventy-five maintenance hemodialysis patients undergoing hemperfusion admitted to the Department of Dialysis,the Second Affiliated Hospital of Guangzhou Uniersity of Chinese Medicine from July to December 2021 were retrospectively selected and divided into traditional hammering method group,one-minute exhaust method group and hammering vibration method group according to perfusion preflush method,with 25 cases in each group.The exhaust time and gas removal effect of three groups of hemoperfusion devices were compared.Results Compared with the three groups,the amount of residual air bubbles in the perfusion apparatus of the one-minute exhaust method group was less than that of the traditional hammering method group and the hammering vibration method group when pre-flushing 200 ml of normal saline,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the amount of residual bubbles between the hammer vibration method group and the traditional hammer vibration method group(P>0.05).The amount of residual air bubbles in the perfusion apparatus in the traditional hammering method group was more than that in the one-minute exhaust method group and the hammering vibration method group when pre-flushing 500 ml of normal saline and 1000 ml of normal saline,with a statistically significant difference(P<0.05).There was no statistically significant difference in the amount of residual bubbles between the one-minute exhaust method group and the hammer vibration method group(P>0.05).There was no statistically significant difference in the amount of residual bubbles among the three groups when pre-flushing 1500ml saline(P>0.05).The exhaust time of the three groups was the shortest in the beating vibration method group,followed by the one-minute exhaust method group,and the longest in the traditional beating method group.The difference between the groups was statistically significant(P<0.05).No preflush anomaly occurred in the three groups.Conclusion The hammering vibration method has the shortest pre-flushing time and satisfactory exhaust effect,while the one-minute exhaust method has the best exhaust effect and satisfactory exhaust effect.
