袋鼠式护理对新生儿重症监护室早产儿亲母母乳喂养的影响研究

Study on the Effect of Kangaroo Care on Parental Breastfeeding of Preterm Infants in the Neonatal Intensive Care Unit

目的:研究袋鼠式护理(KMC)对新生儿重症监护室(NICU)早产儿亲母母乳喂养的影响。方法:选择2019年1月—2020年12月南通大学附属医院NICU收治的80例早产儿作为研究对象,按随机数表法分为对照组和观察组,每组各40例。对照组采用NICU早产儿常规护理,观察组实施KMC。比较两组早产儿产后7 d、14 d、28 d及住院期间的亲母母乳喂养率、喂养量及第1次亲母母乳喂养开始时间、达到全肠喂养时间等母乳喂养情况。结果:观察组产后7 d、14 d、28 d、住院期间的亲母母乳喂养率、喂养量均明显高于对照组,差异有统计学意义(Z=3.212、5.504、3.993、4.506、4.790、3.181、3.560、2.407,P<0.05),观察组第1次亲母母乳喂养开始时间及达到全肠喂养时间均明显短于对照组,差异有统计学意义(Z=2.356、2.224,P<0.05)。结论:KMC应用于NICU早产儿亲母母乳喂养中,能够提高NICU早产儿亲母母乳喂养率和喂养量,缩短第1次亲母母乳喂养开始时间及达到全肠喂养所需时间,促进母乳喂养,改善母婴关系。

Objective:To study the effect of kangaroo mother care(KMC)on parental breastfeeding of preterm infants in the neonatal intensive care unit(NICU).Methods:80 preterm infants admitted to the NICU of the hospital from January 2019 to December 2020 were selected as study subjects and divided into control and observation groups according to the random number table method,with 40 cases in each group.The control group used routine care for preterm infants in the NICU,and the observation group implemented KMC.The breastfeeding rates of the two groups of preterm infants at 7d,14d,28d postnatally and during hospitalization,the amount of feedings and the time of starting 1 parental breastfeeding,and the time of reaching full enteral feeding were compared.Results:The rates of parental breastfeeding and the amount of feeding during 7d,14d,28d,and hospitalization after delivery were significantly higher in the observation group than in the control group,with statistically significant differences(Z=3.212,5.504,3.993,4.506,4.790,3.181,3.560,2.407,P<0.05).The time to start the 1st parental breastfeeding and the time to reach full enteral feeding were significantly shorter in the observation group than in the control group,with statistically significant differences(Z=2.356,2.224,P<0.05).Conclusion:KMC applied to parental breastfeeding of preterm infants in NICU can improve the rate and quantity of parental breastfeeding of preterm infants in NICU,shorten the time to start the first parental breastfeeding and the time required to reach full enteral feeding,which is conducive to promoting breastfeeding and improving mother-infant relationship.
