真武汤合瓜蒌薤白半夏汤加减治疗阳虚水泛型慢性心力衰竭临床观察

Effects of Zhenwu Decoction and Gualou Xiebai Banxia Decoction in Treating Chronic Heart Failure with Yang Deficiency Induced Water Retention

目的探讨真武汤合瓜蒌薤白半夏汤加减治疗阳虚水泛型慢性心力衰竭(CHF)的效果。方法选取上饶市广丰区中医院2018年10月—2021年5月收治的68例阳虚水泛型CHF患者,用随机数字表法分为观察组与对照组,各34例。对照组予以常规西药治疗,观察组加服真武汤合瓜蒌薤白半夏汤加减,连续4周。评估并比较2组治疗前后血清学指标[B型钠尿肽(BNP)、高敏肌钙蛋白T(hs-cTnT)]、心功能指标[左室收缩末期内径(LVESD)、左室舒张末期内径(LVEDD)和左室射血分数(LVEF)]及治疗期间不良反应。结果治疗后,与对照组比较,观察组BNP、hs-cTnT水平及LVESD、LVEDD均较低,LVEF较高(P<0.05);治疗期间2组未见明显不良反应。结论在阳虚水泛型CHF患者中应用真武汤合瓜蒌薤白半夏汤加减治疗,安全可靠,可改善心功能,有利于患者预后。

Objective To investigate the effects of Zhenwu decoction combined with Gualou Xiebai Banxia decoction in the treatment of chronic heart failure(CHF)with yang deficiency induced water retention.Methods 68 patients with CHF admitted to Guangfeng District Hospital of Traditional Chinese Medicine from October 2018 to May 2021 were divided into two groups by random number table method,with 34 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with Zhenwu decoction and Gualou Xiebai Banxia decoction on the basis of the control group.Both groups were treated for 4 weeks.The serum indexes(B-type natriuretic peptide(BNP),high sensitivity troponin T(hs-cTnT)),cardiac function indexes(left ventricular end-systolic diameter(LVESD),left ventricular end-diastolic diameter(LVEDD)and left ventricular ejection fraction(LVEF))and adverse reactions were compared between the two groups.Results Compared with the control group,the levels of BNP,hs-cTnT,LVESD,LVEDD in the observation group were lower and LVEF was higher(P<0.05).There were no obvious adverse reactions in the two groups during treatment.Conclusion Zhenwu decoction combined with Gualou Xiebai Banxia decoction is safe and reliable in the treatment of CHF with yang deficiency induced water retention which can improve the cardiac function and benefit the prognosis of patients.
