支气管肺泡灌洗治疗支气管扩张合并感染的效果

Effect of Bronchoalveolar Lavage on Bronchiectasis Complicated with Infection

目的 探讨支气管肺泡灌洗治疗支气管扩张合并感染的效果。方法 选取2020年6月—2022年6月成武县人民医院收治的60例支气管扩张合并感染患者,所有患者采用随机数字法分为对照组和观察组各30例。对照组采用吸氧支持、全身应用抗菌药物、吸痰、体位引流等常规治疗方案。观察组在对照组治疗方案基础上应用纤维支气管镜支气管肺泡灌洗治疗。比较两组患者的临床疗效、动脉血气情况和血清C-反应蛋白、白介素-6水平等。结果 观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。观察组退热时间、咳嗽消失时间、抗生素应用时间和住院时间均短于对照组,差异均有统计学意义(P<0.05)。治疗后两组动脉血气均较治疗前改善,且观察组氧分压高于对照组、二氧化碳分压低于对照组,差异均有统计学意义(P<0.05)。治疗后两组血清C-反应蛋白和白介素-6水平均低于治疗前,且观察组低于对照组,差异均有统计学意义(P<0.05)。结论 支气管肺泡灌洗治疗支气管扩张合并感染具有较好的疗效,能够促进患者康复,改善动脉血气水平,减轻机体炎症反应,值得推广使用。

Objective To explore the effect of bronchoalveolar lavage in the treatment of bronchiectasis complicated with infection. Methods A total of 60 cases of bronchiectasis complicated with infection admitted to Chengwu County People's Hospital from June 2020 to June 2022 were selected. All patients were randomly divided into control group and observation group with 30 cases in each group. The control group received routine treatment plans, such as oxygen support, systemic application of antibiotics, sputum aspiration, and positional drainage.The observation group was treated with bronchoalveolar lavage using fiberoptic bronchoscopy on the basis of the control group's treatment plan. Compare the clinical efficacy, arterial blood gas status, and serum C-reactive protein and interleukin-6 levels between the two groups. Results The total effective rate of the observation group was significantly higher than that of the control group(P<0.05). The observation group had significantly shorter fever relief time, cough disappearance time, antibiotic application time, and hospitalization time than the control group(P<0.05). After treatment, the arterial blood gas of both groups improved compared to before treatment, and the oxygen partial pressure in the observation group was significantly higher than that in the control group, while the carbon dioxide partial pressure was significantly lower than that in the control group(all P<0.05). After treatment,the average levels of serum C-reactive protein and interleukin-6 in the two groups were significantly lower than before treatment(P<0.05). Conclusion Bronchoalveolar lavage is effective in the treatment of bronchiectasis with infection. It can promote the recovery of patients, improve the level of arterial blood gas, and alleviate the inflammatory reaction of the body, which is worth popularizing.
