摘要
目的:观察针刀治疗Ⅱ~Ⅳ度指屈肌腱狭窄性腱鞘炎的高频超声声像表现及疗效。方法:将72例Ⅱ~Ⅳ度指屈肌腱狭窄性腱鞘炎患者随机分为对照组和观察组,每组36例。对照组采用超短波联合针灸治疗10次,观察组腱鞘炎采用腱鞘炎切开针刀治疗1次,分别对两组患者治疗前及治疗后1周的临床疗效、疼痛指数及手指功能进行评价,并观察治疗前后高频超声声像表现。结果:在高频超声声像表现上,观察组治疗后腱鞘厚度、回声表现及血流信号分级均与治疗前存在统计学差异(P<0.05);总有效率观察组为86.11%(31/36),对照组为58.33%(21/36),观察组高于对照组,差异有统计学意义(P<0.05)。在疼痛指数及手指功能方面,观察组改善程度优于对照组(P<0.05)。结论:腱鞘炎切开针刀治疗Ⅱ~Ⅳ度指屈肌腱狭窄性腱鞘炎疗效满意,可减轻疼痛,改善高频超声声像表现及手指功能。
Objective:To observe high-frequency ultrasonographic features of the treatment of degreeⅡ~Ⅳflexor tendon stenosis tenosynovitis with tenosynovitis incision needle knife and its curative effects.Methods:All 72 patients were randomized into the control group and the observation group,with 36 cases in each group.The control group was treated with ultrashort wave and needling ten times,and the observation group accepted tenosynovitis incision needle knife once,to evaluate clinical effects,pain indexes and finger function before treatment and one week after the treatment respectively,and observe high-frequency ultrasonographic features before and after the treatment.Results:In terms of high-frequency ultrasonographic performance,significant differences were found between tendon sheath thickness,echogenic performance and blood flow signal grading after and before the treatment in the observation group(P<0.05);total effective rate of the observation group was 86.11%(31/36),higher than 58.33%(21/36)of the control group,and the difference had statistical meaning(P<0.05).Regarding pain indexes and finger function,the improvements of the observation group were superior to these of the control group(P<0.05).Conclusion:Tenosynovitis incision needle knife could obtain satisfactory effects in the treatment of tenosynovitis incision needle knife,which could relieve pain,improve high-frequency ultrasonographic performance and finger function.
