超声内镜联合智能分光比色内镜系统检查及Septin9 DNA甲基化检测在低位直肠肿瘤诊断中的应用价值

摘要 目的:研究超声内镜(EUS)联合智能分光比色内镜系统(FICE)检查及Septin9 DNA甲基化检测在低位直肠肿瘤诊断中的应用价值。方法:选取2020年5月—2021年3月惠州市中心人民医院收治的疑似低位直肠肿瘤患者50例作为研究组,选取同期50例健康体检者作为对照组。研究组进行EUS、FICE检查,两组均行Septin9 DNA甲基化检测,比较两组Septin9阳性率,以手术病理诊断结果为“金标准”,分析EUS、FICE诊断结果、诊断效能。结果:研究组Septin9阳性率高于对照组,差异有统计学意义(P<0.001)。FICE检查诊断灵敏度、特异度、准确度低于EUS检查、Septin9 DNA甲基化检测、EUS联合FICE检测,差异有统计学意义(P<0.05)。结论:EUS联合FICE检测与Septin9 DNA甲基化检测能较为准确地诊断低位直肠肿瘤,联合诊断时需要综合多项诊断情况,对患者病灶进行准确判断。 Objective:To study the application value of endoscopic ultrasound(EUS)combined with flexible spectral imaging colour enhancement(FICE)and Septin9 DNA methylation detection in the diagnosis of low rectal tumor.Methods:Fifty patients with suspected low rectal tumor in Huizhou Central People's Hospital from May 2020 to March 2021 were selected as the study group,and 50 healthy subjects for physical examination during the same period were selected as the control group.The study group received EUS and FICE examinations.Both groups received Septin9 DNA methylation detection.The positive rate of Septin9 in the two groups was compared,and the surgical pathological diagnosis results was taken as the golden standard to analyze the diagnostic results and diagnostic efficacy of EUS and FICE.Results:The positive rate of Septin9 in the study group was higher than that in the control group,and the difference was statistically significant(P<0.001).The diagnostic sensitivity,specificity and accuracy of FICE were lower than those of EUS,Septin9 DNA methylation detection and EUS combined with FICE,and the difference was statistically significant(P<0.05).Conclusion:EUS combined with FICE and Septin9 DNA methylation detection can accurately diagnose low rectal tumors.During combined diagnosis,multiple diagnoses need to be combined to accurately determine the lesions of patients.
