基于圆运动理论选穴温针灸治疗以怕冷为主的躯体形式障碍临床研究

A study on the clinical efficacy and safety of warm acupuncture based on circular motion theory in the treatment of somatoform disorders with cold intolerance

目的:观察圆运动理论选穴温针灸治疗以怕冷为主要症状的躯体形式障碍的疗效及安全性。方法:选择重庆三峡医药高等专科学校附属中医院针灸科2020年9月-2021年12月收治的以怕冷为主诉的躯体形式障碍患者共80例,按照随机数字表法分为对照组和治疗组,即基于圆运动理论选穴温针灸治疗(治疗组)和帕罗西汀治疗(对照组),每组各40例。两组均治疗2周。分别于治疗第1周和第2周末观察比较两组临床症状改善情况及症状严重程度指数、症状发生频率指数、症状自评量表(SCL-90)评分。结果:与治疗前相比,治疗1周、2周后治疗组的症状严重程度指数、症状发生频率指数以及SCL-90评分均低于对照组(P<0.05),治疗1周、2周后治疗组的临床总有效率均高于对照组(P<0.05)。结论:基于圆运动理论指导,选择一组固定搭配的穴位行温针灸,通过恢复人体自身圆运动而达到缓解以怕冷为主的躯体形式障碍症状,既避免了药物口服治疗的不良反应,又减轻了因服用神经症类药物给患者带来的心理负担。

Objective:To observe the clinical efficacy and safety of acupuncture based on circular motion theory in the treatment of somatoform disorders mainly due to cold intolerance.Methods:A total of 80 patients with somatoform disorders mainly with cold intolerance were enrolled in the Department of Acupuncture and Moxibustion of the Affiliated Hospital of Chongqing Three Gorges Medical College from September 2020 to December 2021,and were divided into two groups according to the random number table method.They were randomly divided into treatment group(acupuncture based on circular motion theory)and control group(paroxetine),and both groups were treated for 2 weeks.The clinical amelioration of somatic form disorder,symptom severity index,symptom frequency index,and symptom self-rating scale(SCL-90)scores in the two groups was observed at the first week and the end of the second week of treatment.Results:Compared with before treatment,the symptom severity index,symptom frequency index and SCL-90 score in the treatment group after 1 week and 2 weeks of treatment were lower than those in the control group(P<0.05),and the total clinical effective rate in the treatment group after 1 week and 2 weeks of treatment was higher than the control group(P<0.05).Conclusion:Based on the guidance of circular motion theory,a group of fixed acupuncture points for warm acupuncture is selected to alleviate the symptoms of somatoform disorders mainly cold intolerance by restoring the body’s own circular movement,which not only avoids the adverse reactions of oral treatment with drugs,but also reduces the psychological burden brought by taking neurotic drugs.
