社会疏离感在过渡期乳腺癌患者自我表露与孤独感间的中介效应

Mediating effect of social alienation between self-disclosure and loneliness in breast cancer patients during the transitional period

目的探讨社会疏离感在过渡期乳腺癌患者自我表露与孤独感间的中介效应。方法便利抽取2021年10月至2022年10月就诊于河南省人民医院的317例过渡期乳腺癌患者为研究对象。采用一般资料调查表、癌症孤独量表、一般疏离感量表及痛苦表露指数量表进行调查。采用SPSS22.0分析过渡期乳腺癌患者社会疏离感、孤独感及自我表露现状及相关性。采用Process插件的Model 4分析过渡期乳腺癌患者社会疏离感在孤独感与自我表露间的中介效应,同时采用Bootstrap法对中介效应进行检验。结果过渡期乳腺癌患者孤独感得分为(22.27±6.75)分,社会疏离感总分为(38.12±8.82)分,自我表露总分为(36.79±11.49)分;经Pearson相关性分析及Bootstrap中介效应分析,结果显示,孤独感与自我表露呈负相关(r=-0.413,P<0.001),孤独感与社会疏离感呈正相关(r=0.405,P<0.001),自我表露与社会疏离感呈负相关(r=-0.379,P<0.001);社会疏离感在自我表露与孤独感间起部分中介作用,中介效应占总效应的26.75%。结论自我表露可能通过社会疏离感间接影响过渡期乳腺癌患者孤独感,提示医护人员可通过提高自我表露水平,降低社会疏离感,以减轻过渡期乳腺癌患者的孤独感。

Objective To explore the mediating effect of social alienation between self-disclosure and loneliness in breast cancer patients during the transitional period.Methods A total of 317 breast cancer patients during the transitional period admitted to Henan Provincial People's Hospital from October 2021 to October 2022 were selected as the research objects.Using the convenient sampling method,a total of 317 breast cancer patients were selected as the research subjects.The general information questionnaire,Cancer Loneliness Scale,General Alienation Scale,and Distress Disclosure Index were used for investigation.SPSS22.0 was used to analyze the status and correlation of social alienation,loneliness,and self-disclosure in transitional breast cancer patients.Model 4 of process plug-in was used to analyze the mediating effect of social alienation between loneliness and self-disclosure in transitional breast cancer patients,and Bootstrap method was used to test the mediating effect.Results The score of loneliness,social alienation,and self-disclosure was(22.27±6.75)points,(38.12±8.82)points,and(36.79±11.49)points,respectively.Through Pearson correlation analysis and Bootstrap mediation effect analysis,the analysis results showed that loneliness was negatively correlated with self-disclosure(r=-0.413,P<0.001).Loneliness was positively correlated with social alienation(r=0.405,P<0.001).Self-disclosure was negatively correlated with social alienation(r=-0.379,P<0.001).Social alienation played a partial mediating role in the relationship between self-disclosure and loneliness,and the mediating effect accounted for 26.75%.Conclusion Self-disclosure can indirectly affect loneliness through the intermediary effect of social alienation.Medical workers should take positive measures to reduce loneliness by improving patients’self-disclosure and reducing social alienation.
