下肢静脉溃疡患者营养状况评估及影响因素分析

Assessment of nutritional status of patients with lower limb venous ulcers and analysis of infl uencing factors

目的 采用COUNT评分评估下肢静脉溃疡患者的营养状况,分析下肢静脉溃疡患者营养不良的影响因素。方法 收集2018年1月—2021年12月入院诊断为下肢静脉溃疡163例患者为调查对象,将COUNT评分为0~1分的57例患者作为营养正常组(对照组);COUNT评分>1分的106例患者为营养不良组(病例组)。在单因素分析的基础上,采用多因素logistic回归分析下肢静脉溃疡患者营养不良的影响因素。结果 单因素分析显示,营养不良组患者年龄、糖尿病比例、下肢骨关节疾病比例、肺气肿比例、PA、TG、TP、HGB、NLR值与营养正常组相比差异具有统计学意义(P<0.05)。多因素Logistic回归分析显示,NLR为下肢静脉溃疡患者营养不良的独立危险因素,而PA和TG则为保护因素。结论 COUNT评分可以快速有效识别下肢静脉溃疡患者营养不良状况,NLR为下肢静脉溃疡患者营养不良的独立危险因素,而PA和TG则为保护因素。

Objective To assess the nutritional status of patients with lower limb venous ulcers by taking the COUNT score and analyse the infl uencing factors of malnutrition in patients with lower limb venous ulcers.Methods 163 patients admitted to the hospital and diagnosed with lower limb venous ulcers from January 2018 to December 2021 were collected as investigation subjects,and 57 patients whose COUNT score were 0~1 points were taken as the nutritionally normal group(control group);106 patients whose COUNT score were>1 points were taken as the malnutrition group(case group).On the basis of univariate analysis,multifactorial logistic regression was used to analyse the infl uencing factors of malnutrition in patients with lower limb venous ulcers.Results Univariate analysis showed that,the diff erences in age,proportion of diabetes mellitus,proportion of lower limb osteoarthritis,proportion of emphysema,PA,TG,TP,HGB,and NLR values of patients in the malnourished group were statistically significant compared with those of the nutritionally normal group(P<0.05).Multifactorial logistic regression analysis showed that,NLR was an independent risk factor for malnutrition in patients with lower limb venous ulcers,while PA and TG were the protective factors.Conclusion COUNT score can quickly and eff ectively identify the malnutrition status of patients with lower extremity venous ulcers,and NLR is an independent risk factor for malnutrition in patients with lower extremity venous ulcers,while PA and TG are protective factors.
