Ⅱ~Ⅲ期结直肠癌患者根治术后肿瘤标志物水平检测及其对复发的预测价值

Detection of Tumor markerslevels and their predictive value for recurrence in patients with stageⅡ~Ⅲcolorectal cancer after radical surgery

目的探讨Ⅱ~Ⅲ期结直肠癌患者根治术后血清癌胚抗原(CEA)、糖类抗原199(CA19-9)、甲胎蛋白(AFP)、血管生成素-2(Ang-2)、血管内皮生长因子(VEGF)水平检测及其对复发的预测价值。方法回顾性分析2019年1月至2021年1月在浙江省杭州市富阳区中医院行根治术治疗的76例Ⅱ~Ⅲ期结直肠癌患者临床资料并将其设为观察组,术后随访1年,根据有无复发分为复发组(n=32)和无复发组(n=44)。同期选择本院体检中心健康人员60例设为健康组。检测所有人员的血清CEA、CA19-9、AFP、Ang-2、VEGF水平,并用Logistic回归分析其对结直肠癌患者术后早期复发的预测价值。结果观察组患者的血清CEA、CA19-9、AFP、Ang-2、VEGF水平均高于健康组(P<0.05);复发组患者的血清CEA、CA19-9、AFP、Ang-2、VEGF水平明显高于无复发组(P<0.05);Logistic回归分析显示:血清CEA、CA19-9、AFP、Ang-2、VEGF水平是导致Ⅱ~Ⅲ期结直肠癌患者术后早期复发的独立影响因素(P<0.05)。结论血清CEA、CA19-9、AFP、Ang-2、VEGF水平与结直肠癌发生、术后复发有关,上述指标联合检测对辅助诊断Ⅱ~Ⅲ期结直肠癌患者术后早期复发有较理想价值。

Objective To explore the detection of serum levels of carcinoembryonic antigen(CEA),carbohydrate antigen 199(CA19-9),alpha fetoprotein(AFP),angiopoietin-2(Ang-2),and vascular endothelial growth factor(VEGF)in patients with stageⅡ-Ⅲcolorectal cancer after radical surgery,and their predictive value for recurrence.Methods A retrospective analysis was conducted on the clinical data of 76 patients with stageⅡtoⅢcolorectal cancer who underwent radical surgery in our hospital from January 2019 to January 2021.They were divided into recurrence group(n=32)and non-recurrence group(n=44)based on postoperative follow-up for 1 year.During the same period,60 healthy personnel from our hospital's physical examination center were selected as the health group.The serum levels of CEA,CA19-9,AFP,Ang-2,and VEGF were detected in all personnel,and their predictive value for early postoperative recurrence in colorectal cancer patients was analyzed using logistic regression.Results The serum levels of CEA,CA19-9,AFP,Ang-2,and VEGF in the observation group were higher than those in the healthy group(P<0.05).The serum levels of CEA,CA19-9,AFP,Ang-2,and VEGF in the recurrent group were significantly higher than those in the non-recurrent group(P<0.05).Logistic regression analysis showed that serum levels of CEA,CA19-9,AFP,Ang-2,and VEGF were independent influencing factors for early postoperative recurrence in patients with stageⅡ-Ⅲcolorectal cancer(P<0.05).Conclusion The levels of serum CEA,CA19-9,AFP,Ang-2,and VEGF are related to the occurrence and postoperative recurrence of colorectal cancer,and the combined detection of these indicators has ideal value in assisting in the diagnosis of early postoperative recurrence in patients with stageⅡ-Ⅲcolorectal cancer.
