贫血相关指标、铁代谢指标及炎症指标与铁调素的相关性分析

Correlation of anemia related indexes, iron metabolism indexes and inflammatory indexes with Hepcidin

目的 研究贫血相关指标、铁代谢指标及炎症指标与铁调素的相关性。方法 回顾性选择2016年5月至2018年1月于保定市第二中心医院接受治疗的血液病患者84例,按照是否贫血分为贫血组(n=32)、非贫血组(n=52)。选择同期于保定市第二中心医院行健康体检的正常人45名纳入对照组。检测并观察3组血浆铁调素水平,红细胞计数(RBC)、红细胞平均血红蛋白浓度(MCHC)、红细胞平均体积(MCV)、血红蛋白水平等贫血相关指标水平。分析血清铁、不饱和铁结合力(UIBC)、铁蛋白等铁代谢指标水平,白细胞介素(IL)-1、IL-6和肿瘤坏死因子α(TNF-α)等炎症指标水平与铁调素的相关性。结果 3组血浆铁调素水平及RBC、MCHC、MCV、血红蛋白水平比较,差异均有统计学意义(P<0.05);贫血组血浆铁调素水平及MCV均较非贫血组、对照组高,RBC、MCHC、血红蛋白水平均较非贫血组、对照组低,非贫血组血浆铁调素水平及MCV均较对照组高,RBC、MCHC、血红蛋白水平均较对照组低,差异均有统计学意义(P<0.05)。3组血清血清铁、UIBC、铁蛋白水平比较,差异均有统计学意义(P<0.05);贫血组血清血清铁、UIBC、铁蛋白水平均较非贫血组、对照组高,非贫血组血清血清铁、UIBC、铁蛋白水平均较对照组高,差异均有统计学意义(P<0.05)。3组血清TNF-α、IL^(-1)、IL-6水平比较,差异均有统计学意义(P<0.05);贫血组血清TNF-α、IL^(-1)、IL-6水平均较非贫血组、对照组高,非贫血组血清TNF-α、IL^(-1)、IL-6水平均较对照组高,差异均有统计学意义(P<0.05)。经Pearson相关分析,MCV、血清铁、UIBC、铁蛋白及TNF-α、IL^(-1)、IL-6与铁调素呈正相关,血红蛋白水平、RBC与铁调素呈负相关(P<0.05)。MCHC与铁调素不相关(P>0.05)。结论 血液病贫血患者存在贫血相关指标、铁代谢指标、炎症指标及铁调素异常,且多数贫血相关指标、铁代谢指标、炎症指标与铁调素具有相关性。

Objective To study the Correlation of anemia related indexes,iron metabolism indexes and inflammatory indexes with Hepcidin.Methods A total of 84 patients with hematological diseases treated in Baoding Second Central Hospital from May 2016 to January 2018 were selected and divided into anemia group(n=32)and non-anemia group(n=52)according to whether they were anemic or not.Forty-five normal people who underwent health checkup in Baoding Second Central Hospital during the same period were included in the control group.Plasma hepcidin level,anemia-related indexes such as red blood cell count count(RBC),mean corpuscular hemoglobin concentration(MCHC),mean corpuscular volume(MCV),hemoglobin level of the three groups were detected and observed.The correlation between serum iron metabolism indexes such as serum iron,unsaturated iron binding capacity(UIBC),ferritin,inflammatory indexes such as interleukin(IL)-1,IL-6 and tumor necrosis factor-α(TNF-α)and hepcidin were analyzed.Results The plasma hepcidin level,RBC,MCHC,MCV and hepcidin level,were different among the three groups,the differences were statistically significant(P<0.05).Plasma hepcidin level and MCV in anemia group were higher than those in non-anemia group and control group,RBC,MCHC and hepcidin level,were lower than those in non-anemia group and control group,the plasma hepcidin level and MCV in non-anemic group were higher than those in control group,while RBC,MCHC and hepcidin level,were lower than those in control group,the differences were statistically significant(P<0.05).There were differences in serum iron,UIBC and FERR levels among three groups,the differences were statistically significant(P<0.05).The levels of serum iron,UIBC and FERR in anemia group were higher than those in non-anemia group and control group,the levels of serum iron,UIBC and FERR in non-anemic group were higher than those in control group,the differences were statistically significant(P<0.05).The levels of serum TNF-α,IL^(-1) and IL-6 were different among three groups,the differences were statistically significant(P<0.05).The levels of serum TNF-α,IL^(-1) and IL-6 in anemia group were higher than those in non-anemia group and control group,the levels of TNF-α,IL^(-1) and IL-6 in the non-anemic group were higher than those in the control group,the differences were statistically significant(P<0.05).By Pearson correlation analysis,MCV,SI,UIBC,FERR,TNF-α,IL^(-1),IL-6 were positively correlated with hepcidin,while hepcidin level and RBC were negatively correlated with hepcidin(P<0.05).There was no correlation between MCHC and hepcidin(P>0.05).Conclusion Anemia patients have anemia-related indexes,iron metabolism indexes,inflammatory indexes and hepcidin abnormalities,moreover,most anemia-related index,iron metabolism index,and inflammation index were correlated with hepcidin.
