中药饮片处方的临床用药情况分析

Analysis of Clinical Use of TCM Decoction Pieces Prescription

目的:分析中药饮片处方的临床用药情况。方法:选取2020年3月—2023年2月绥阳县人民医院研究药剂科500张中药饮片处方作为研究对象,进行处方点评,分析500张中药饮片处方的用药情况,总结不合理处方类型。结果:500张中药饮片处方中,41~60岁患者用药最多;中药处方治疗疾病类型以脾胃科、中医康复科、肝病科疾病比较常见,500张中药处方中脾胃科占比最高,为38.40%,其余依次为中医康复科(29.20%)、肝病科(15.80%)、骨科(7.60%)、心血管科(5.60%)及妇科(3.40%);每剂药量以<100 g占比最高,为43.80%,其次为100~200 g、201~300 g、>300 g;科室内处方用药味数最多为16~20味,占比40.00%,其余依次为6~10味(20.00%)、11~15味(18.20%)、>20味(13.20%)和1~5味(8.60%);应用有毒饮片51例,占比10.20%;用药不合理处方类型包括信息录入错误、临床诊断不规范和使用方法错误3种类型,分别占比6.40%、4.00%、1.00%。结论:在对中药饮片处方管理过程中,需加强对中药饮片处方的管理质量,降低不合理用药发生率,提升用药规范性和安全性。

Objective:To analyze the clinical use of TCM decoction pieces prescription.Methods:Five hundred prescriptions of TCM decoction pieces in the Department of Research and Pharmacy of Suiyang County People's Hospital from March 2020 to February 2023 were selected as the research objects for prescription review.The situation of five hundred TCM decoction pieces prescription was analyzed,and the types of unreasonable prescription were summarized.Results:Among the 500 prescriptions of TCM decoction pieces,patients aged 41 to 60 used the most drugs;The diseases of spleen and stomach,rehabilitation of traditional Chinese medicine and liver disease were more common,among the 500 TCM prescriptions,spleen and stomach accounted for the highest proportion(38.40%),the rest of the order for the rehabilitation of traditional Chinese medicine(29.20%),liver(15.80%),bone(7.60%),cardiovascular(5.60%)and gynecological(3.40%);The proportion of<100 g per dose was the highest(43.80%),followed by 100-200 g,201-300 g and>300 g;The maximum number of prescription tastes was 16-20,accounting for 40.00%,followed by 6-10(20.00%),11-15(18.20%),>20(13.20%)and 1-5(8.60%);Toxic decoction pieces were used in 51 cases,accounting for 10.20%;The types of irrational drug use prescriptions included information entry error,non standard clinical diagnosis and wrong use method,accounting for 6.40%,4.00%and 1.00%respectively.Conclusion:In the process of prescription management of TCM decoction pieces,it is necessary to strengthen the management quality of TCM decoction pieces prescription,reduce the incidence of irrational drug use,and improve the standardization and safety of drug use.
