医患护陪共同决策护理模式对脑外伤后睡眠障碍患者疾病不确定感及睡眠质量的影响

Effect of Doctor-patient Co-decision-making Nursing Model on Disease Uncertainty and Sleep Quality in Patients with Post-traumatic Sleep Disorder

目的:分析医患护陪共同决策护理模式对脑外伤后睡眠障碍患者疾病不确定感及睡眠质量的影响。方法:选取2019年1月至2021年6月中国人民解放军联勤保障部队第九一〇医院收治的脑外伤后睡眠障碍患者128例作为研究对象,按照随机数字表法分为对照组和观察组,每组64例。对照组采用常规护理干预,观察组采用医患护陪共同决策护理模式干预,比较2组患者的疾病不确定感(疾病不确定感评分),睡眠质量[匹兹堡睡眠质量指数(PSQI)评分]及负面情绪[汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)评分]。结果:干预后,观察组患者的疾病不确定感总分、复杂性评分以及不明确性评分均低于对照组,差异均有统计学意义(均P<0.05);观察组患者的PSQI各项指标评分及总分均低于对照组,差异均有统计学意义(均P<0.05);观察组患者的HAMD评分、HAMA评分均低于对照组,差异均有统计学意义(均P<0.05)。结论:医患护陪共同决策护理模式可明显减轻脑外伤后睡眠障碍患者的疾病不确定感,提高睡眠质量,缓解负面情绪。

Objective:To analyze the effect of doctor-patient co-decision-making nursing model on the disease uncertainty and sleep quality of patients with post-traumatic sleep disorder.Methods:A total of 128 patients with sleep disorder after brain trauma admitted to the 910th Hospital of the People′s Liberation Army Joint Logistic Support Force from January 2019 to June 2021 were selected as the research objects,and were divided into the control group and the observation group according to the random number table method,with 64 cases in each group.The control group used routine nursing intervention,and the observation group used doctor-patient co-decision-making nursing model The uncertainty of disease(uncertainty of disease score),sleep quality[Pittsburgh Sleep Quality Index(PSQI)score]and negative emotions[Hamilton Depression Scale(HAMD),Hamilton Anxiety Scale(HAMA score)]of the two groups were compared.Results:After intervention,the total score of disease uncertainty,complexity score,and uncertainty score of the observation group were lower than those of the control group,with statistically significant differences(P s<0.05).The PSQI scores and total scores of the observation group were lower than those of the control group,and the differences were statistically significant(P s<0.05).The HAMD score and HAMA score of the observation group patients were lower than those of the control group,and the differences were statistically significant(P s<0.05).Conclusion:The nursing mode of doctor-patient co-decision-making can obviously reduce the uncertainty of disease,improve the quality of sleep and relieve the negative emotion.
