摘要
目的:分析上海市奉贤区南桥镇社区卫生服务中心采用的全科医师培训教学模式对全科医师诊疗能力的影响。方法:选取2020年4月—2023年1月在奉贤区南桥镇社区卫生服务中心接受中山-南桥全科培训教学模式的30名全科医师为调查对象,采用简单随机抽样方法将全科医师分为观察组和对照组各15名。两组全科医师均接受中山-南桥全科培训教学模式,培训前后分别采用客观结构化临床考核方法进行总结性评估,观察组定期采用小型临床演练评量、操作技能直接观察评量等进行形成性评估。结果:培训前两组全科医师的全科接诊能力、技能操作能力、小讲课能力得分差异均无统计学意义(P>0.05),培训后观察组的全科接诊能力、技能操作能力、小讲课能力得分均高于对照组,差异有统计学意义(P<0.05)。培训后观察组的小型临床演练评量、操作技能直接观察评量所有项目得分均高于培训前,差异有统计学意义(P<0.05)。结论:形成性评估与总结性评估相结合的培训考核方法能够更为综合全面地评价学员的学习效果,在南桥社区全科医师培训教学模式中取得了较好的效果,值得在社区全科教学中进行推广。
Objective:To analyze the impact of the general practitioner training and teaching mode adopted by Nanqiao Community Health Service Center in Fengxian District,Shanghai on the diagnostic and treatment abilities of the general practitioners.Methods:Thirty general practitioners who received Zhongshan-Nanqiao general practice training and teaching mode at Nanqiao Community Health Service Center in Fengxian District from April 2020 to January 2023 were selected as the survey subjects.A simple random sampling method was used to divide the general practitioners into an observation group and a control group with 15 each.Both groups of the general practitioners received the Zhongshan-Nanqiao general practice training and teaching mode,and objective structured clinical assessment methods were used for summary evaluation before and after the training,the observation group regularly used small-scale clinical practice evaluation,direct observation evaluation of operational skills,and other formative evaluations.Results:There was no statistically significant difference in the scores of general practice consultation ability,skill operation ability,and small lecture ability between the two groups of the general practitioners before the training(P>0.05).After the training,the scores of general practice consultation ability,skill operation ability,and small lecture ability in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).After training,the observation group scored higher in all items of small-scale clinical practice evaluation and direct observation evaluation of operational skills than before training,and the differences were statistically significant(P<0.05).Conclusion:The training assessment method combining formative assessment and summative assessment can comprehensively evaluate the learning effectiveness of students,and has achieved good results in the training and teaching mode of the general practitioners in Nanqiao Community,which is worth promoting in the community general practice teaching.
