以行为转变理论为指导的康复护理对膝关节骨性关节炎患者的影响

Effect of Rehabilitation Nursing Guided by Behavior Change Theory in Patients with Knee Osteoarthritis

目的:探讨以行为转变理论为指导的康复护理对膝关节骨性关节炎(KOA)患者的影响。方法:选取2020年12月1日~2022年12月31日96例KOA患者按随机数字表法分为观察组和对照组各48例,对照组采用常规护理,观察组在对照组基础上实施以行为转变理论为指导的康复护理。比较两组干预前后康复效果[采用美国膝关节协会评分量表(KSS)、Berg平衡量表(BBS)]、疼痛程度[采用视觉模拟评分法(VAS)]、自我管理行为(采用自我管理行为量表)及社会功能[采用社会功能缺陷筛查量表(SDSS)]。结果:干预后,两组KSS、BBS评分高于干预前(P<0.05),且观察组高于对照组(P<0.05);干预后,两组静息和运动状态下VAS评分低于干预前(P<0.05),且观察组低于对照组(P<0.01);干预后,两组自我管理行为量表各维度评分高于干预前(P<0.05),且观察组高于对照组(P<0.05);干预后,两组SDSS评分低于干预前(P<0.05),且观察组低于对照组(P<0.01)。结论:对KOA患者实施以行为转变理论为指导的康复护理,可提高康复效果和患者的自我管理行为能力,促进其社会功能恢复。

Objective:To explore the effect of rehabilitation based on change theory rehabilitation effect and social function of in patients with knee osteoarthritis.Methods:A total of 96 patients from December 1,2020 to December 31,2022 were divided into the observation group and the control group by the random number table method,with 48 cases in each group.The control group was given routine nursing,while the observation group was given rehabilitation nursing guided by behavior change theory on the basis of the control group.Rehabilitation effect by the American Knee Association Scale(KSS)and Berg Scale(BBS),pain degree by the Visual Analogue Scale(VAS),self-management behaviors by the self-management scale and social function by the social deficiency screening scale(SDSS)were compared between the two groups before and after intervention.Results:After the intervention,the scores of the American Knee Association Scale(KSS)and Berg Scale(BBS)in both groups were higher than those before the intervention(P<0.05),amd those in the observation group were higher than those in the control group(P<0.05);and the scores of the Visual Analogue Scale(VAS)were significantly lower in both resting and exercise states(P<0.01).After intervention,the self-management scale scores in both groups were significantly higher than those before intervention(P<0.05),and those in the observation group were higher than those in the control group(P<0.01).After intervention,the scores of the Social Function Deficiency Screening Scale(SDSS)in both groups were significantly lower than those before intervention(P<0.05).And those in the observation group were significantly lower than those in the control group(P<0.01).Conclusion:The application of intervention nursing guided by behavior change theory in KOA patients can effectively improve rehabilitation effect and their self-management behaviors,which is beneficial to recovery of their social function.
