关节镜下微骨折术联合氨甲环酸关节腔注射用于半月板损伤对术后疼痛及关节功能的影响

Effects of arthroscopic microfracture surgery combined with intra-articular injection of tranexamic acid on postoperative pain and joint function in meniscus injury

目的 探究应用关节镜下微骨折术联合氨甲环酸关节腔注射疗法对半月板损伤患者术后疼痛程度和膝关节功能的影响。方法 选取本院2020年4月至2021年11月期间收治的半月板损伤患者100例作为研究对象,并按随机分组法分成对照组(50例)和研究组(50例)。对照组应用关节镜下微骨折术治疗,研究组在关节镜下微骨折术治疗的基础上结合关节腔注射氨甲环酸治疗,随访时长为3个月。比较两组的关节活动范围及肌力、膝关节疼痛程度(VAS)评分、膝关节相关量表评分[Lysholm膝关节功能量表(LKS)、Tegner膝关节运动量表(TKMS)]和术后合并症发生率等的变化。结果 两组治疗后的关节活动范围和股四头肌肌力均高于本组治疗前(P<0.05),且研究组治疗后的指标数据均高于对照组(P<0.05);两组治疗后的VAS评分均低于本组治疗前(P<0.05),且研究组治疗后的评分均低于对照组(P<0.05);两组治疗后的膝关节活动范围、LKS、TKMS评分均高于本组治疗前(P<0.05),且研究组治疗后的活动范围和评分均高于对照组(P<0.05);研究组术后合并症的发生率为2.00%低于对照组的14.00%(P<0.05)。结论 关节镜下微骨折术联合氨甲环酸关节腔注射的疗法对半月板损伤患者的修复效果显著,能提高关节活动范围和肌力水平,缓解术后疼痛,改善膝关节功能情况,减少术后合并症的发生。

Objective To explore the effects of arthroscopic microfracture surgery combined with intra-articular injection of tranexamic acid on postoperative pain and joint function in meniscus injury.Methods A total of 100 patients with meniscus injury admitted to the hospital were enrolled as the research objects between April 2020 and November 2021.According to random grouping method,they were divided into control group(50 cases)and study group(50 cases).The control group was treated with arthroscopic microfracture surgery,while study group was additionally given intra-articular injection of tranexamic acid.All were followed up for 3 months.The changes in range of motion,muscle strength,scores of visual analogue scale(VAS)and knee related scales[Lysholm Knee Scale(LKS),Tegner Knee Motion Scale(TKMS)],and incidence of postoperative complications were compared between the two groups.Results After treatment,range of motion and muscle strength of quadriceps were increased in both groups(P<0.05),which were higher in study group than control group(P<0.05).After treatment,VAS scores were decreased in both groups(P<0.05),which were lower in study group than control group(P<0.05),while knee range of motion,LKS and TKMS scores were increased in both groups(P<0.05),which were higher in study group than control group(P<0.05).The incidence of postoperative complications in study group was lower than that in control group(2.00%vs 14.00%,P<0.05).Conclusion Arthroscopic microfracture surgery combined with intra-articular injection of tranexamic acid has significant repair effects on patients with meniscus injury,which can improve range of motion and muscle strength,relieve postoperative pain,improve knee function and reduce postoperative complications.
