叙事护理联合围手术期快速康复对前列腺癌患者术后心理应对方式及控尿功能的影响

Effect of Narrative Nursing Combined with Perioperative Rapid Rehabilitation on Postoperative Psychological Coping Style and Urinary Control Function in Patients with Prostate Cancer

目的探讨叙事护理联合围手术期快速康复对前列腺癌患者术后心理应对方式及控尿功能的影响。方法前瞻性纳入郑州大学第一附属医院2021年1月至2022年10月拟接受根治术治疗的82例前列腺癌患者为研究对象,通过随机数字表法将其分为对照组与观察组,每组41例。对照组接受围手术期快速康复护理,观察组在围手术期快速康复护理基础上接受叙事护理,两组均干预2周。比较两组干预2周时控尿能力(尿失禁分度);比较两组干预前、干预2周时两组心理应对方式(简易应对方式问卷评分)及盆底肌肉锻炼依从性[盆底肌肉锻炼自我效能量表(BPMSES)评分]。结果干预2周时,观察组控尿能力优于对照组(P<0.05);干预2周时,两组积极应对评分均明显上升,且观察组高于对照组,消极应对评分下降,且观察组低于对照组(P<0.05);干预2周时,两组BPMSES评分均上升,且观察组高于对照组(P<0.05)。结论叙事护理联合围手术期快速康复可使前列腺癌患者以积极应对方式面对疾病,可提高盆底肌肉锻炼依从性和控尿能力。

Objective To explore the effect of narrative nursing combined with perioperative rapid rehabilitation on postoperative psychological coping styles and urinary control function in patients with prostate cancer.Methods A prospective study was conducted on 82 patients with prostate cancer who were scheduled to undergo radical surgery in the First Affiliated Hospital of Zhengzhou University from January 2021 to October 2022.They were randomly divided into control group and observation group,with 41 patients in each group.The control group received perioperative rapid rehabilitation nursing,while the observation group received narrative nursing based on perioperative rapid rehabilitation nursing,both groups were intervened for 2 weeks.The ability of urinary incontinence control(urinary incontinence score)between the two groups after 2 weeks of intervention was compared.Psychological coping style(simple coping style questionnaire score)and pelvic floor muscle exercise compliance[Broome pelvic muscle self-efficacy scale(BPMSES)score]were compared between the two groups before intervention and 2 weeks after intervention.Results After 2 weeks of intervention,the urine control ability of the observation group was better than that of the control group(P<0.05).After 2 weeks of intervention,the positive coping scores of the two groups increased significantly,and the observation group was higher than the control group,the negative coping scores decreased,and the observation group was lower than the control group(P<0.05).After 2 weeks of intervention,the BPMSES scores of both groups were significantly increased,and the observation group was higher than the control group(P<0.05).Conclusion Narrative nursing combined with perioperative rapid recovery can enable prostate cancer patients to face the disease in a positive manner,improve pelvic floor muscle exercise compliance and urinary control ability.
