308 nm准分子激光联合卤米松乳膏治疗白癜风患者的效果

Effects of 308 nm excimer laser combined with Halometasone cream in treatment of vitiligo patients

目的:观察308 nm准分子激光联合卤米松乳膏治疗白癜风患者的效果。方法:选取2020年2月至2022年3月该院收治的114例白癜风患者进行前瞻性研究,按照随机数字表法将其分为对照组与观察组各57例。对照组采用卤米松乳膏治疗,观察组在对照组基础上联合308 nm准分子激光治疗。比较两组临床疗效,治疗前和治疗1、3个月后皮损色素积分、皮损面积、T细胞亚群指标水平,以及不良反应发生率。结果:观察组治疗总有效率为94.74%(54/57),高于对照组的82.46%(47/57),差异有统计学意义(P<0.05);治疗1、3个月后,观察组皮损色素积分高于对照组,皮损面积小于对照组,差异有统计学意义(P<0.05);治疗1、3个月后,观察组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均高于对照组,CD8^(+)水平低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P<0.05)。结论:308 nm准分子激光联合卤米松乳膏治疗白癜风患者可提高治疗总有效率和皮损色素积分,改善T细胞亚群指标水平,减小皮损面积,效果优于单纯卤米松乳膏治疗。

Objective:To observe effects of 308 nm excimer laser combined with Halometasone cream in treatment of vitiligo patients.Methods:A prospective study was conducted on 114 patients with vitiligo admitted to this hospital from February 2020 to March 2022.They were divided into control group and observation group according to the random number table method,57 cases in each group.The control group was treated with Halometasone cream,while the observation group was treated with 308 nm excimer laser on the basis of that of the control group.The clinical efficacy,the pigment integral of skin lesions,the skin lesion area,the T cell subgroup index levels,and the incidence of adverse reactions were compared between the two groups before and 1 and 3 months after the treatment.Results:The total effective rate of treatment in the observation group was 94.74%(54/57),which was higher than 82.46%(47/57)in the control group,and the difference was statistically significant(P<0.05).After 1 and 3 months of treatment,the pigment integral of skin lesions in the observation group was higher than that in the control group,the skin lesion area was smaller than that in the control group,and the differences were statistically significant(P<0.05).After 1 and 3 months of treatment,the levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in the observation group were higher than those in the control group,the level of CD8^(+)was lower than that in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P<0.05).Conclusions:308 nm excimer laser combined with Halometasone cream can improve the total effective rate and the pigment integral of skin lesions,improve the levels of T cell subsets and reduce the skin lesion area in the patients with vitiligo.Moreover,it is superior to single Halometasone cream treatment.
